This document is a Past Performance Questionnaire used by the Department of Veterans Affairs to evaluate an Offeror's past performance on a similar contract. The questionnaire is intended to gather feedback from references provided by the Offeror regarding their performance on a contract for replacing floor services.

• The questionnaire asks for an assessment of the Offeror's performance in several areas, including:
  • Quality of performance (overall and in specific areas like conformance to contract requirements and standards of workmanship).
  • Management performance (overall ability to meet contract requirements and specific areas like responsiveness, personnel management, program management, and conflict resolution).
  • Cost control (overall ability to forecast, manage, and control costs, and specific areas like estimates and invoicing).
• Rating options include Exceptional, Satisfactory, Unsatisfactory, and Not Applicable.
• Narrative comments are encouraged, especially for "Exceptional" or "Unsatisfactory" ratings.

This questionnaire is a critical component of the evaluation process for the referenced opportunity. Bidders should be aware that their past performance will be assessed by references they provide. The feedback gathered through this questionnaire will directly impact the evaluation of their proposal. The information provided is considered confidential and exempt from Freedom of Information Act release.

Supplemental Instructions / Other

This document provides supplemental instructions for offerors responding to solicitation (SF1449) (Page 1) for an Indefinite Delivery, Indefinite Quantity (IDIQ) contract with an estimated period of performance from 1/19/2026 to 1/18/2031.

• Quotes must be submitted by the due date and time specified in Box 8 of the SF1449.

• Late quotes and phone calls will not be accepted.

• Quotes and questions must be submitted via email only to Jameel Gordon (jameel.gordon@va.gov).

• Quotes must comply with all aspects of the SF1449 (RFQ) to be considered.

• Point of Contact: Patrice d'Entremont (Patrice.D'Entremont@va.gov). Bidders running more than 15 minutes late should contact the POC.

• Site Visit Day 1 (12/18/2025):

  • Lexington Park CBOC (9:00 AM – 11:30 PM EST): 45870 East Run Drive, Suite 300, Lexington Park, MD 20653. Meet at the main entrance.
  • Charlotte Hall CBOC (1:00 PM – 3:30 PM EST): 29640 Three Notch Road, Charlotte Hall, MD 20622. Meet at the main entrance.

• Site Visit Day 2 (12/19/2025):

  • Montgomery County CBOC (9:00 AM – 11:30 PM EST): 15810 Gaither Dr, Gaithersburg, MD 20877. Meet at the main entrance.
  • Washington DC VA Medical Center (Main) & Fisher House (1:00 PM – 3:30 PM EST): 50 Irving Street, Northwest, Washington, DC 20422-0001. Meet in the hospital lobby.

• SF1449 Completion: Complete Boxes 12, 17a., and 30a., b., and c. Include entity name, address, SAM UEI, and cage code in Box 17a. Complete Section B.1 (Contractor POC) and Section B.3 (Price/Cost Schedule) with unit prices for each CLIN.

• Technical Capability: Submit documentation describing the methods for performing the work.

• Pricing: Enter legible unit prices in the price/cost schedule.

• Past Performance: Provide a minimum of three (3) years of relevant past performance, including CPARS records or a completed Past Performance Questionnaire (See Section D, Attachment).

• Fill-ins: Complete all applicable provisions or clauses incorporated by reference per VAAR 852.252-70.

Solicitation / RFP / RFQ

This solicitation is a request for quote (RFQ) for a Firm Fixed Price contract for plant maintenance and holiday décor services for the Washington DC VA Medical Center and its outlying Community Based Outpatient Clinics (CBOCs). The contract will be an Indefinite Delivery, Indefinite Quantity (IDIQ) type, with a five-year period of performance.

• Interior Plants: Maintenance, replacement, and care of interior plants.

• Holiday Decorations: Rental of holiday décor.

• Locations: Washington DC VA Medical Center (Main Campus), Fisher House, Montgomery County CBOC, Lexington Park CBOC, and Charlotte Hall CBOC.

• Vendor Experience: Minimum of 3 years providing ancillary services in healthcare/corporate environments, including horticulture knowledge, NFPA 101 décor requirements, and Joint Commission standards.

• Maintenance Tasks: Includes initial audit, yearly deficiency lists, weekly maintenance, watering, pruning, dusting, fertilization, insect/disease control, and removal of old plants.

• Holiday Decor: Use for no more than two holiday seasons, installation/removal by vendor, and storage by vendor.

• Contract Type: Firm Fixed Price, Indefinite Delivery, Indefinite Quantity (IDIQ).

• Period of Performance: Estimated from 01/19/2026 to 01/18/2031.

• Minimum Guaranteed Amount: $2,500.00.

• Maximum Contract Ceiling: $500,000.00.

• Offer Submission: Signed and dated offers to be submitted by 01-05-2026 at 11:00 AM EST.

• Questions Deadline: Written questions due by 12/23/2025 at 11:00 AM EST to jameel.gordon@va.gov.

• Evaluation Factors: Lowest Price Technically Acceptable (LPTA). Technical and past performance are equally important.

• NAICS Code: 561730 (Landscaping Services).

• Size Standard: $9.5 Million.

• The acquisition appears to be set aside for small business, with specific provisions for HUBZone, 8(a), Service-Disabled Veteran-Owned, Economically Disadvantaged Women-Owned, or Women-Owned Small Business concerns.

• The solicitation includes FAR clauses 52.212-1, 52.212-3, and 52.212-5.
