Statement of Work (SOW) / Performance Work Statement (PWS)

To provide preventative maintenance drainage system services for the Northern Navajo Medical Center (NNMC) SR0001 and the West Wing Administration Building SR0004, ensuring the removal of waste matter to maintain healthy conditions in the buildings' main drainage system.

• Bi-annual cleaning of drain lines in problem areas.

• Jetting of drain lines for two 1-week visits.

• Bio-Clean treatment for drain lines.

• High-resolution video camera inspections to assess drain line conditions, identify blockages, cracks, or deterioration.

• Routine inspections and cleaning of sewer lines.

• Ensuring waste from faucets, toilets, and drains is managed appropriately.

• Use of mechanical augers for build-up.

• Development and maintenance of a comprehensive safety program, including adherence to OSHA requirements for Personal Protective Equipment (PPE).

• Training and recommendations for NNMC Maintenance Staff on drain line maintenance.

• Provision of a completed Service Report for all preventative maintenance services.

• Immediate repair of any damages caused during services.

• Maintenance of work areas in a clean and orderly condition.

• Base period: 12 months.

• Option periods: Four (4) periods of 12 months each.

• Contractor must be licensed to perform plumbing drain line and other plumbing services in the State of New Mexico.

• Work must be coordinated with Facility Management with 120 hours (5 business days) prior notice to minimize disruption to patient care.

• Contractor shall comply with Hospital Policy, including Pre-construction Risk Assessments (PRCA), Interim Life Safety Measure (ILSM), and Infection Control Risk Assessments (ICRA).

• Contractor shall provide all labor, tools, materials, and necessary supervision.

• Contractor personnel must obtain a visitor badge after checking in with Facility Maintenance.
