This solution will support various specialty care services including Emergency Medicine, Obstetrics/Gynecology, Critical Care, Anesthesia, Family Medicine, Inpatient Wards, Internal Medicine, Pediatrics, Sports Medicine, Orthopedics, General Surgery, Urology, Endocrinology, Rheumatology, Nephrology, Neurology, Pain Management, Gastroenterology, and Otolaryngology (ENT). The information gathered will facilitate decision-making within DHA and identify qualified, experienced, and interested potential business sources.
Information Requested
Respondents are requested to submit a Capability Statement (limited to 10 pages) addressing:
- General and technical experience within the last five years, specifically with contracts similar in scope, complexity, and size.
- Federal Government performance-based experience, including contract details, price, period of performance, and project description.
- Program management plan and control policies demonstrating project tracking and timely completion.
- A draft implementation plan outlining the potential solution, key deliverables, timelines, and risk mitigation strategies.
- Company information including Cage Code, Unique Entity Identifier (UEI), name, address, point of contact, structure, Small Business Designation status (e.g., 8(a), HUBZone, SDVOSB), applicable NAICS codes, GSA SINs, and other contract numbers (e.g., GSA OASIS+, MAS, CIO-SP3).
