Statement of Work (SOW) / Performance Work Statement (PWS)

To provide insurance coverage for Portuguese civilian employees of the United States employed by United States Forces Azores (USFORAZORES) at Lajes Field, Terceira, Azores. This coverage includes job-related accidents/injuries and accidents/injuries or unexpected illnesses during Temporary Duty (TDY) status.

• Provide all necessary personnel, equipment, tools, materials, supervision, transportation, and safety equipment.
