This solicitation is a Request for Proposals (RFP) for locally employed staff health insurance coverage for the U.S. Embassy in Lusaka, Zambia. The government is seeking off-the-shelf plans that meet minimum benefit levels, with the intent to award a contract to the responsible company submitting an acceptable offer at the lowest price. Initial proposals may lead to award without discussions, though discussions may be held if necessary.

• Provision of health insurance services to employees of the U.S. Embassy in Lusaka and their eligible dependents.

• Coverage must meet minimum benefit levels as described in Section C.

• Specific benefits include, but are not limited to: Hospitalization, Emergency Services, Ambulance, Outpatient Services, Obstetric and Newborn Care, Pediatric Services, Prescription Drugs, Preventive and Wellness Services, Hearing Aids, Optical Care, Dental Care, Reproductive Health, Mental Health and Substance Abuse Care, Rehabilitative and Habilitative Services, HIV/AIDS, and Catastrophic Coverage.

• The contract will be a fixed-price with economic price adjustment type.

• Period of Performance: One year beginning 30 days following contract award, with four (4) one-year options to renew.

• Estimated minimum guarantee: 2 premiums per period of performance.

• Estimated maximum: 8000 premiums for the base and all option years.

• Value Added Tax (VAT) will not be reimbursed; the contractor must not include VAT on invoices due to the Embassy's tax exemption.

• Pre-proposal conference: Tuesday, May 5, 2026, at 10:00 AM at the US Embassy. Contact lusaka-procurement@state.gov by April 30, 2026, for information or to arrange entry.

• Proposal Submission Deadline: Wednesday, May 20, 2026, at 5:00 PM CAT.

• Proposals must be in English and submitted electronically to lusaka-procurement@state.gov.

• Accepted file formats: MS-Word 2007/2010, MS-Excel 2007/2010, or Adobe Acrobat (pdf).

• File size limit: 30MB per file; separate files must be used for larger submissions.

• Required submission documents: SF-33, Section B (Pricing Schedule and Retention Amounts), Section K (Representations and Certifications), and proof of SAM Registration.

• Award will be made to the lowest-priced, technically acceptable, responsible offeror.

• Offerors must be registered in the System for Award Management (SAM) at https://www.sam.gov prior to proposal submission.

• Eligible participants are U.S. Mission employees and their dependents in Lusaka, Zambia, as well as ORE Staff and Peace Corps employees.

• The solicitation includes numerous FAR and DOSAR clauses covering various aspects of contract performance, compliance, and administration.

• Exhibits A, B, and C provide employee statistics and claims data for informational purposes.
