This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as defined by the Buy Indian Act and HHSAR 326.601. This form is required in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Self-Certification: Offerors must attest whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Eligibility Period: The enterprise must meet the definition throughout the offer period, at contract award, and during the contract performance period. Immediate notification is required if eligibility is lost.
• Required Information: The form requires the Offeror to provide:
  • Name of Federally Recognized Tribal Entity (if applicable)
  • Name of the 51% owner of the IEE Firm
  • Business Name
  • Unique Entity Identifier (UEI) Number
  • Certifying Signature and Print Name

This form is crucial for bidders responding to opportunities that are set-aside or sole-sourced under the Buy Indian Act. Failure to complete this form accurately or meet the eligibility requirements can lead to disqualification or penalties. Offerors must be prepared to provide documentation of eligibility if requested and must be registered in the System of Award Management (SAM).

Sources Sought Notice

This notice is for market research purposes by the Indian Health Service (IHS), Department of Health and Human Services (HHS), to determine if a 100% Set Aside for Small Business Indian Firms or Small Business concerns can fulfill the requirements for "Other Computer Related Services" (NAICS Code: 541519). This is a Request for Information (RFI) only and not a solicitation for proposals.

• Item/Service: 1 (ea.) printer/scanner/copier for the Mescalero Service Unit DSFC and DEHS.
• Contract Inclusions: Base year plus a 3-year extended warranty and maintenance plan.
• Place of Performance: Indian Health Service, Mescalero Service Unit, Environmental Health Building, 436 Mescal Loop, Mescalero, NM 88340.

The IHS will give preference to Indian economic enterprises in accordance with the Buy Indian Act. Responses will help determine the acquisition strategy, which could include Indian-Owned Enterprise set asides, Total Small Business aside, other Socio-Economic set asides, or an Unrestricted acquisition.

Interested firms must submit the following in writing:

1. Company name.
2. Point of contact (name/title/telephone number/e-mail address).
3. FSS / SEWP Contract number, if applicable.
4. SAM Unique Entity Identifier Number.
5. Tax ID number.
6. Indication of business category (IEE or ISBEE) for which the organization qualifies.
7. A capability statement addressing qualifications and ability to provide the requirement.
8. For Indian Economic Enterprises (IEE), the attached IHS Indian Economic Enterprise Representation form, self-certifying eligibility and identifying the Federally Recognized Indian Tribe or Alaska Native Corporation.

Statement of Work (SOW) / Performance Work Statement (PWS)

The Indian Health Service (IHS), Office of Environmental Health and Engineering (OEHE) requires the purchase and maintenance of one printer/scanner/copier for the Mescalero Service Unit DSFC and DEHS.

• Provide and install one (1) printer/scanner/copier.

• Include a base year plus a 3-year extended warranty and maintenance plan.

• Perform 6-month service, cleaning, and maintenance visits by a factory-certified technician (total of 8 visits over 4 years).

• Provide up to 2 on-call service visits per year for troubleshooting and repairs.

• Black & white and color printing (letter and 11x17 sizes, 2-sided).

• Black & white and color copying (letter and 11x17 sizes, 2-sided).

• Color scanning (letter and 11x17 sizes, 2-sided).

• Network connectivity for the IHS OEHE network.

• Total contract duration: 4 years from the Notice to Proceed (NTP) date (1 base year + 3 additional years for maintenance).

• Location: Indian Health Service, Mescalero Service Unit, Environmental Health Building, 436 Mescal Loop, Mescalero, NM 88340.

• Installation and deployment of the equipment.

• For each 6-month maintenance visit: A report summarizing work performed, personnel involved, and an invoice.

• The Contracting Officer's Representative (COR) will evaluate each maintenance visit.
