This document is the "Indian Economic Enterprise Representation Form" required by the Buy Indian Act. It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" (IEE) as defined by HHSAR 326.601. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Representation: Offerors must indicate whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act by checking the appropriate box ("does" or "does not").
• Name of Federally Recognized Tribal Entity: Field for the name of the tribal entity.
• Name of the 51% owner of IEE Firm: Field for the name of the 51% owner.
• Business Name: Field for the business name.
• Certifying Signature: Field for the signature of the certifying individual.
• Unique Entity Identifier (UEI) Number: Field for the Unique Entity Identifier.
• Print Name: Field for the printed name of the certifying individual.

This form is crucial for bidders seeking to participate in opportunities designated under the Buy Indian Act. Bidders must accurately complete and submit this form to self-certify their eligibility as an Indian Economic Enterprise. Failure to meet the definition or provide accurate information can lead to disqualification or penalties. Offerors must also understand that the IEE status must be maintained throughout the contract performance period, and any changes must be immediately reported. Registration with the System of Award Management (SAM) is also a requirement for successful offerors.

Statement of Work (SOW)

To procure an annual renewal of 3M Advanced Analyzer Software, specifically 3M Health Information Systems for coding and reimbursement resources for the Quentin N. Burdick Memorial Health Care Facility in Belcourt, North Dakota.

• Provide 3M licensing fees for Coding and Reimbursement Software.

• Software includes ICD-10 Coding references, DRG-finder, CPT-finder, Clinical Analyzer, and DRG Reimbursement Calculations.

• Includes a Coding Reference Library with various medical dictionaries and coding guides.

• Services are commercial, severable, and provided as a fixed-price, non-personal contract.

• Contractor is responsible for maintaining adequate resources, equipment, and patient confidentiality (adhering to HIPAA and HITECH).

• Contractor must provide instructions and training on software usage.

• Accuracy, clarity, and consistency to requirements in all work products.

• Work products must be editable by the Government and submitted in hard copy and electronic copy.

• General quality measures will be applied to all work products.

• Task order for a 12-month period, with four (4) option periods.

• Estimated start date: 4/1/2026 – 3/31/2027.

• Services to be provided remotely and/or contact COR (TBD) via email.

• Contractor must have extensive knowledge in providing Coding and Reimbursement resource systems.

• Must meet Federal Acquisition Regulation (FAR) Part 9 contractor qualifications (financial resources, performance record, integrity, organization, technical skills, equipment, and eligibility).

• Compliance with Section 508 Electronic and Information Technology Accessibility is required, including submission of an HHS Section 508 Product Assessment Template.

• Adherence to all applicable Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation (HHSAR) clauses.
