Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for Community Nursing Home (CNH) services to be provided to Veterans by authorized medical center personnel. The goal is to ensure care meets the health needs and promotes the maximum well-being of VA patients.

• Provide nursing home care, including room, meals, nursing care, and other services commensurate with the VA-authorized level of care.

• Ensure facilities are certified by CMS (Medicare/Medicaid) and hold a state nursing home license.

• Cooperate with VA staff in referring appropriate Veterans and accepting referrals in a timely fashion (within 24 hours of request).

• Offer provider visits (one per month), laboratory, x-ray, and other special services as needed.

• Motivate and educate patients to achieve and maintain independence in daily living activities.

• Arrange specialty care services for Veterans, such as dental, podiatry, and ophthalmology.

• Manage medical records in accordance with the Privacy Act of 1974 and CMS standards.

• VA will provide quality oversight visits to ensure continuity of care and assist in Veterans' transition back to the community.

• CNHs must cooperate with VA's quality improvement or quality assurance program functions.

• Timely development of Corrective Action Plans (CAPs) for identified deficiencies.

• Reportable events (e.g., sentinel events, changes in ownership/administration, substantiated allegations of mistreatment, infectious outbreaks, adverse events) must be reported to the VA CNH Coordinator within 24 hours or the first business day after a weekend/holiday.

• This is an Indefinite Delivery Indefinite Quantity (IDIQ) contract with an ordering period of up to five years.

• Services are to be provided to the U.S. Department of Veterans Affairs Palo Alto Health Care System in Palo Alto, CA.

• Facilities must have current CMS certification and a state nursing home license.

• Adherence to HIPAA compliance is required.

• VA reserves the right to remove any or all VA patients from the CNH at any time.

• VA will pay a bed hold for re-hospitalizations or therapeutic passes, with limitations.

• CNHs will not directly solicit charitable contributions from Veteran patients or their families.

• Invoices for services must be submitted by the 15th calendar day of the following month.

• VA payments constitute the total cost of nursing home care; no additional charges will be billed to other entities or individuals without prior written authorization from the VA.
