Statement of Work (SOW) / Performance Work Statement (PWS)

To provide acute bedside dialysis services to eligible patients at the Boise VA Medical Center (BVAMC) as the facility does not have internal dialysis services.

• Provide Registered Nurse (RN) services for acute bedside dialysis.

• Services include Hemodialysis, Peritoneal Dialysis, and Continuous Renal Replacement Therapy (CRRT).

• Contractor must furnish all necessary equipment and supplies (e.g., CRRT cartridges, PD/HD solutions, tubing, needles, cleaning supplies).

• Contractor is responsible for equipment maintenance, repair, and regular preventative maintenance.

• Contractor must clean equipment after each use according to manufacturer recommendations.

• Contractor must maintain logs for cleaning and maintenance.

• Contractor must submit semi-annual reports on equipment maintenance, downtime, and safety certifications.

• Contractor must submit quarterly reports on a Performance Improvement Plan.

• Contractor must demonstrate a compliance level of no less than 90% in areas including vascular access site maintenance, dialysis prescription adherence, pain assessment/reassessment, patient education documentation, and hand-off communication.

• Contract Base Year: December 1, 2025, through November 30, 2026, with four one-year option periods.

• Personnel Qualifications: RNs must hold a current, valid, and unrestricted license in a U.S. state, territory, or commonwealth. Must have current Basic Life Support (BLS) certification, negative PPD/chest x-ray, and Hepatitis B immunity or declination. Must complete VA VetPro credentialing. BSN preferred but not required.

• Training: Contractor staff must complete BLS, competency verification, VAMC orientation modules, VA Privacy and Information Security Awareness training, and HIPAA training.

• Location: Offeror must be located within a 50-mile radius of the Boise VA Medical Center.

• Security: Contractor personnel require background investigations (NACI level clearance) and must adhere to VA security and privacy protocols. Liquidated damages apply for data breaches involving Sensitive Personal Information (SPI).

• Insurance: Contractor must provide worker's compensation and professional liability insurance.

• Reporting: Submit weekly or monthly invoices through the Invoice Payment Processing System (IPPS). Submit semi-annual maintenance reports and quarterly performance improvement reports.

• Appearance: Contractor employees must wear agency ID badges, maintain professional decorum, and wear closed-toed shoes. Scrub suits are permitted.
