Statement of Work (SOW) / Performance Work Statement (PWS)

To provide Virtual Pharmacy Services to Veterans Integrated Service Network 04 (VISN 04) facilities to ensure medication orders are safe and accurate, thereby increasing efficiency and improving the delivery of medication orders.

• Provide drug knowledge resources and pharmacist medication order review and entry services.

• Review authorized physician medication orders transmitted or accessed by the contractor.

• Evaluate prescriptions for drug interactions, allergies, dose appropriateness, route, administration time, and clarity.

• Clarify pending medications with the prescriber if they do not meet standards.

• Utilize the VA National Formulary (VANF) for medication selection.

• Provide daily shift "sign off" reports summarizing outstanding issues.

• Inpatient and Outpatient VISTA Pharmacy package order review.

• Triage and prioritize pending orders: Stat, emergent, and ER orders must be completed within 15 minutes of entry (95% of the time).

• All other orders must be completed within 30 minutes of entry (95% of the time).

• Emergency orders and consultations must be processed within 15 minutes of contact.

• Develop contingency plans for VISTA system shutdowns.

• Comply with all applicable policies, rules, and regulations of each VAMC.

• Comply with Joint Commission (TJC) standards.

• Complete and input all transcription notes directly into VISTA/EHR/CPRS/Cerner.

• Provide a contact for customer service complaints, available within 24 hours.

• Contract Period: April 1, 2026, to March 31, 2031.

• Services provided to VISN 04 facilities: Lebanon, PA VAMC; Altoona, PA VAMC; Butler, PA VAMC; Coatesville, PA VAMC; Erie, PA VAMC; Wilkes-Barre, PA VAMC; and, Wilmington, DE VAMC.

• Contractor must be a United States-based company.

• Pharmacists must have access to VISTA, EHR, CPRS/Cerner.

• Contract is for non-personal services.

• Pharmacists must have active, unrestricted licenses, be U.S. citizens, speak English proficiently, and have professional degrees.

• Must undergo a Low Risk, Tier-1 background investigation.

• Must complete VA Privacy and Information Security Awareness training.

• Must comply with credentialing and privileging processes (VetPro).

• Provide general orientation for all contract pharmacists.

• Adhere to HIPAA regulations.

• Provide a roster of contract pharmacists and updated staff information quarterly.

• Deliver a Provider Exit Review within seven days of employee departure.
