Statement of Work (SOW) / Performance Work Statement (PWS)

To provide on-site Microwave Ablation (MWA) and Cryoablation equipment and technical assistance to support Interventional Radiologists at the Omaha VA Nebraska-Western Iowa Healthcare System (VA NWIHCS) to maximize patient safety.

• Provide a trained and experienced technician to assist the VA Interventional Radiologist.

• Furnish all needed mobile MWA/Cryoablation equipment and supplies.

• Provide technical expertise and guidance during procedures.

• Services include management, training, labor, time, transportation, equipment, mobile ablation equipment, tools, supplies (excluding gases), maintenance, and repair of contractor-furnished equipment.

• Ensure all ablation equipment is FDA approved and in good working order.

• Contractor is responsible for procurement, dispensing, administration, and safekeeping of all required supplies.

• Response Time: MWA/Cryoablation unit must be on-site and in the CT Suite at least 60 minutes prior to the scheduled start time. Vendor must provide staff and equipment 60 minutes prior to the scheduled time. Acceptable Quality Level (AQL): 95%.

• Mandatory Training: Contractor staff must complete all mandatory VA training, including HIPAA/Privacy, Fire Safety, Infection Control, and Bloodborne Pathogens. AQL: 100%.

• Standard of Care: Services must be provided in accordance with civilian medical treatment facility standards and VA NWIHCS policies for patient safety and quality assurance.

• Compliance: Adherence to all applicable VHA and VA NWIHCS policies, procedures, regulations, and Joint Commission standards.

• Period of Performance: Base contract period is November 1, 2020, through October 31, 2021, with four one-year option periods, exercisable at the government's discretion.

• Place of Performance: On-site at the Omaha VA NWIHCS, primarily in the Imaging Department, located at 4101 Woolworth Avenue, Omaha, NE 68105.

• Quantities: Estimated yearly requirements. Minimum guaranteed quantity is 15 procedures; maximum is 400 procedures (base year plus any option years).

• Personnel Qualifications: Technicians must be trained and experienced with ablation equipment (minimum one year experience) and proficient in written and spoken English. Must have completed required VA training (HIPAA/Privacy, security, ethics, fire prevention).

• Security: Personnel require security background checks, fingerprinting for S.C. clearance, and completion of eQIP for a low-risk NACI background investigation. ID badges/PIV cards will be issued.

• Business Associate Agreement (BAA): Required.

• Information Security: Strict adherence to VA information security and privacy regulations is required.

• Billing: Invoices must be submitted electronically via Tungsten Network by the 20th workday of the month following services. Payment is made to the contractor only; no billing to beneficiaries or third parties is allowed.

Notice of Intent to Sole Source

This notice announces the Department of Veterans Affairs' intent to award a sole source contract for On-Site Ablation Equipment and Technical Assistance. This service is required to support the Omaha NE VA Hospital's Interventional Radiologists in performing Microwave Ablation (MWA) and Cryoablation procedures, aiming to maximize patient safety.

• Furnish all personnel, supplies (excluding gases), and equipment for MWA and Cryoablation procedures during normal business hours.
