Scope of Work
The contractor will provide on-site Occupational Therapy Services for students at San Ildefonso Day School, supporting the implementation of special education and related services under the Individualized with Disabilities Education Improvement Act (IDEIA). Services include observation, screening, evaluation, documentation, developing and implementing therapy plans, maintaining student records, identifying adaptive equipment needs, coordinating with school personnel and parents, and assisting with IEP development. Services are estimated for 2-4 students per year, approximately 4 hours per week. Occupational Therapy Reports are required within 5 working days of request.
Contract Details
- Contract Type: Labor Hour purchase order.
- Period of Performance: A base year with four (4) one-year option periods. The exact period will be updated upon award.
- Pricing: All-inclusive occupational therapy service rate, paid per hour.
- Set-Aside: Unrestricted/Full and Open competition.
- Key Personnel Requirements: Master's Degree in Occupational Therapy, at least 5 years of experience with Native American students, recommended National Board Certification, and a New Mexico State License. All personnel require a favorable screening and background investigation by the BIA and OPM.
