• Removed Clauses: Clause 52.219-30 (Notice of Set-Aside for, or Sole-Source Award to, Women-Owned Small Business Concerns Eligible Under the Women-Owned Small Business Program) has been removed.

This is a combined synopsis/solicitation for commercial products or services, specifically requesting quotes for Physical Therapy Service for San Felipe Pueblo Elementary School (SFES). This announcement is the only solicitation, and a written solicitation will not be issued.

Statement of Work (SOW) / Performance Work Statement (PWS)

To procure the staffing services of a Physical Therapist (PT) for the San Felipe Pueblo Elementary School to provide physical therapy evaluation and compensatory services to students.

• Perform physical therapy evaluations for students requiring compensatory services.

• Meet with and advise special education and classroom teachers on student assessment, evaluation, and motor therapy program implementation.

• Meet with individual students regularly to provide suggestions for home-based therapy strategies.

• Perform tasks in accordance with IDEA, Section 504, and 25 Code of Federal Regulations Part 39 – Indian School Equalization Program (ISEP).

• Complete comprehensive review and summarize student records.

• Conduct evaluations and assessments adhering to minority nondiscriminatory practices.

• Provide evaluation reports within two weeks of examination.

• Meet with students for regular weekly therapy sessions, individually or in groups, as determined by the IEP/placement committee.

• Log in/out of school site and schedule for IEP committee meetings.

• Submit brief progress reports every 4 weeks for each student.

• Meet with parents/guardians for consultation regarding student progress.

• Maintain records of work performed for monthly invoicing, including a log of therapy sessions and a mileage log.

• Evaluation reports due within two weeks of examination.

• Progress reports submitted every 4 weeks.

• Hours of Operation: Monday - Friday, 7:30 am to 4:00 pm MDT, excluding holidays and school closures. Minimum of 7 hours per day required.

• Experience: Minimum 2 years as a Physical Therapist, with experience working in Native American communities preferred.
