This document provides supplemental instructions for offerors responding to Solicitation (SF1449) / RFQ for Radiation Oncology Services. It clarifies submission requirements, evaluation criteria, and contract details.

• Offerors must complete SF1449 (Page 1) Boxes 12, 17a., and 30a., b., and c.

• Provide entity name, address, telephone number, SAM UEI, and cage code in Box 17a.

• Complete Section B.1. for Contractor POC information.

• Complete Section B.3 Price/Cost Schedule with unit prices for each CLIN.

• Evaluation of options under FAR 52.217-8 will consider the possibility of exercising options in one to six-month increments, not exceeding a total of six months. The evaluation will assume option prices will be the rates in effect at the time of exercise.

• A Comparative Evaluation process will be used, which is a direct comparison of quotations to determine the "best" overall response in a uniform and fair manner. It is not an LPTA or trade-off process.

This solicitation is for Radiation Oncology Assessment Services for the National Radiation Oncology Program (NROP).
