Statement of Work (SOW) / Performance Work Statement (PWS)

To acquire Continuous Readiness (CR) services for the Bay Pines VA Healthcare System to ensure ongoing compliance with The Joint Commission (TJC) and Ascellon standards. The goal is to enhance the healthcare system's readiness, improve processes, increase patient safety, and educate staff on accreditation survey procedures.

• Conduct a 5-day Mock TJC survey at each Bay Pines VA Healthcare System facility.

• Include one Life Safety consultative visit per facility.

• Focus surveys on frequently cited standards, problematic areas, and high-risk areas such as Environment of Care and Life Safety.

• Identify organizational and systems issues related to safety, quality of care, and inter-process interfaces.

• Utilize the most current TJC and Ascellon standards applicable to Hospital, Behavioral Health, Long Term Care, and Home Care.

• Provide an exit briefing and a written report detailing findings, specific noncompliance citations, and recommendations for resolution within fifteen (15) working days after the site visit.

• Surveys must adhere to "The Joint Commission, Survey Activity Guide for Health Care Organizations All Accreditation Programs."

• Contractor consultants must possess a minimum of three years of surveying experience and specific knowledge of TJC, Ascellon, and VHA regulations, with a preference for previous VA experience.

• A written report citing specific standards of noncompliance, narrative assessment, and recommendations is required.

• Contractor performance will be monitored by VA personnel, with summary reports submitted to the COR and CO.

• Performance Period: One Year

• Locations: Bay Pines VA Healthcare System and its associated facilities (listed in Attachment A).

• Consultants must have experience in Life Safety processes and be knowledgeable of challenges faced by VA Medical Centers.

• Contractor personnel are subject to Federal laws and VA directives regarding information security.

• A National Business Associate Agreement (BAA) is required.

• No substitution of surveyors is permitted without VA CO consultation. Unsatisfactory performance may result in a request for contractor replacement at no additional cost to the VA.

Sources Sought Notice

This notice is a market research tool to identify potential qualified sources capable of providing "Mock Life Safety – Joint Commission Survey" services at the Bay Pines VA Healthcare System. It is not a solicitation for bids or proposals. The government is gathering information to determine the appropriate acquisition method.

Respondents are requested to provide the following:

• Business size status under NAICS code 541690.

• Existing government contract numbers for similar scopes, including expiration dates.

• Percentage of the total contract value intended for subcontracting.

• Identification of work to be performed by in-house staff versus subcontractors.
