Scope of Work

The selected contractor will perform surveys aligned with the Centers for Medicare and Medicaid Services (CMS) process, identifying deficient practices in SVHs. Key tasks include:

• Developing and managing survey schedules.
• Conducting unannounced, onsite surveys.
• Preparing and submitting various reports, including Annual Survey Reports, Survey Deficiency Summary Reports, and Ad Hoc Reports.
• Participating in Kick-Off, Monthly, and Quarterly meetings.
• Ensuring compliance with detailed regulatory standards for Nursing Home, Domiciliary, and Adult Day Health Care programs, as outlined in Quick Reference Guides and Life Safety Checklists.

Contract Details

• Contract Type: Indefinite Delivery/Indefinite Quantity (IDIQ) Firm Fixed Price (FFP).
• Period of Performance: A 12-month base period and four (4) 12-month option periods.
• Minimum Guarantee: $40,000 for the base period.
• Set-Aside: Women-Owned Small Business (WOSB) Program Set-Aside (FAR 19.15).
• NAICS Code: 541611 (Administrative Management and General Management Consulting Services) with a $24.5 Million size standard.
