Scope of Work
The contractor shall provide comprehensive document destruction services, including:
- Secure collection and destruction of confidential paper records containing Personally Identifiable Information (PII) and Protected Health Information (PHI).
- Provision of secure, locked collection containers (32-gallon, 64-gallon, and 95-gallon equivalents).
- Scheduled and on-demand collection services.
- On-site shredding using equipment capable of rendering materials unreadable and irrecoverable; off-site destruction is not authorized.
- Maintenance of a secure chain-of-custody for all materials.
- Issuance of Certificates of Destruction (COD) for each service visit, detailing date, location, container ID, time, method, and weight of materials destroyed. Services will be performed at various VA medical centers and associated facilities throughout North Florida and South Georgia.
