All questions received on January 21, 2026, will be posted to SAM.gov by the close of business on that same day.

• The hour and date specified for receipt of Offers is extended.

• Proposal deadlines have been extended as noted above. Offerors must acknowledge receipt of this amendment.

Questions & Answers / Clarifications

This document provides answers to vendor questions regarding the Request For Quotes (RFQ) for the Glasgow VA Clinic (36C25926Q0182).

• Lab Processing: Labs will be processed by the offeror, not the clinic.
• Order Submission: The clinic will submit orders detailing the quantity and type of tests needed.
• Supplies: The clinic will not supply any supplies to the offeror.
• Guaranteed Volume: There is no guaranteed volume; services are provided as needed by the VA.
• Availability: Offerors shall be available when services are needed by the VA, with notice provided as needed.
• Staffing Fees: All staffing fees must be included in the proposal's price totals.
• Deliveries: No supplies are delivered to the clinic.
• Incumbent: The incumbent contractor is Frances Mahon Deaconess Hospital.
• Supply Orders: Supply orders are placed in the clinic but shared with the offeror.
• Test Completion: Tests are completed by the offeror's lab, with results available to the clinic.
• Contract Scope: The contract is neither for supplies nor live persons.
• Phlebotomist: The clinic is not asking for a phlebotomist, but for a hospital/clinic capable of performing the tests listed on the RFQ.

These clarifications provide essential details for potential bidders regarding service execution, cost inclusions, and the scope of work. Bidders should ensure their proposals reflect these answers, particularly concerning the offeror's responsibility for processing, supplies, and staffing fees.

Solicitation / RFP / RFQ

This solicitation is for Reference Laboratory Testing services for Veterans assigned to the Glasgow Community Based Outpatient Clinic (CBOC) in Glasgow, Montana. The contract will provide emergent and routine laboratory sample collection and testing when VA staff are unavailable or immediate results are needed. This is a Firm-Fixed-Priced Open-Market Blanket Purchase Agreement (BPA).

• Provide laboratory tests authorized by a VA physician.

• Services to be performed within the city limits of Glasgow, Montana.

• Strict patient identification procedures (at least two identifiers).

• Collection and testing of samples that cannot be shipped (e.g., Blood Gas Analysis, Ammonia levels, cultures).

• Transmission of verified results via fax to Glasgow VA Clinic and VA Fort Harrison Laboratory.

• Contractor must be accredited by CAP, CLIA, or JC.

• Personnel must be licensed and competent.

• Services are nonpersonal in nature.

• Contract Type: Firm-Fixed-Priced Open-Market Blanket Purchase Agreement (BPA).

• Period of Performance: February 10, 2026, through February 9, 2031 (Base Year + 4 Ordering Periods).

• Estimated individual BPA Orders: Up to $50,000.00 per Order.

• Authorized Ceiling Limit: $200,000.00 over the agreement period.

• Proposals due: January 21, 2026, by 9:00 AM MT.

• Clarification questions due: January 19, 2026, by 10:00 AM MT.

• Evaluation Factors: Technical (Pass/Fail), Past Performance, and Price.

• Technical Factor requires demonstration of meeting SOW requirements, capability for listed tests, and proof of CLIA certification.

• Past Performance will be assessed via CPARS and PPIRS, with references required.

• Price will be evaluated for reasonableness based on the Cost Per Test (CPT) schedule.

• Award will be made to the responsible offeror whose offer is most advantageous to the Government, price and other factors considered.

• This acquisition is unrestricted.

• Site visits are not allowed.

• Construction is defined as moving walls, drains, or permanent columns; site readiness procedures are allowed.

• Offerors must make pricing and terms valid for a minimum of 120 calendar days from the solicitation closing date.
