Statement of Work (SOW)

To manage the removal, transportation, and disposal of regulated medical waste (RMW) generated by the Crow Service Unit (CSU) healthcare facility, ensuring compliance with all relevant federal and state regulations.

• Provide a service contract for RMW management.

• Transport RMW to an approved mitigation facility in compliance with DOT regulations.

• Ensure disposal or destruction adheres to EPA and DEQ guidelines.

• Handle the removal, transportation, and disposal of used sharp instruments (needles, scalpels, lancets, burs, scissors).

• Provide CSU with necessary containers and hardware for secure mounting or storage, meeting DOT and ANSI standards.

• Conduct training sessions for CSU Housekeeping staff on DOT Hazardous Materials and Biohazard Waste.

• Perform removal and proper disposal of all RMW.

• Conduct two (2) removal trips per month.

• Supply necessary RMW containers, sufficient for two weeks of use.

• Remove sharps boxes twice per month.

• Replace used sharps containers twice per month.

• First pick-up within 14 days of award.

• Containers and hardware supplied within 14 days of award.

• Mounting/securing hardware supplied within 30 days of award.

• Manifest completed at time of service.

• Invoices submitted within 30 days of service.

• Place of Performance: Crow/Northern Cheyenne Hospital, 10110 South, 7650 East Crow Agency, Montana 59022.

• Period of Performance: One (1) base year plus four (4) one-year option periods.

• No IT or facility clearances are needed.

Bidders must complete and submit this form to self-certify their eligibility as an "Indian Economic Enterprise" if the solicitation or opportunity requires it under the Buy Indian Act. Failure to meet the definition or properly complete the form may impact eligibility for award. Offerors are also reminded that they must maintain this status throughout the contract performance period and must be registered with SAM. False representations are subject to legal penalties.
