This document is a template for bidders to complete and submit with their quotation for the CAHA – Debris Removal project at Cape Hatteras National Seashore. It is used to assess technical acceptability on a Lowest Price Technically Acceptable (LPTA) basis.

• Quoter Information: Requires legal name, UEI, CAGE, Primary POC details (name, title, email, phone).
• Relevant Experience: Requires details on two (2) completed debris removal projects within the last five years, including customer, location, scope, timing, contract number (if applicable), and POC information.
• Key Subcontractor Identification: Requires identification of subcontractors and their roles if work will be subcontracted.
• SOW/Specs Compliance Checklist: Requires bidders to confirm compliance with specific requirements, including mobilization time, hazardous material handling, personnel qualifications, and completion within 21 days. It also requires acknowledgment of the period of performance (45 calendar days from award notice) and identification of any known schedule constraints.
• Quoter Certification: Requires an authorized representative's signature, name, title, and date, certifying the accuracy of the provided information and compliance with the SOW.

Completion of this template is mandatory for a quotation to be rated Technically Acceptable. Failure to include all required entries will result in a "FAIL" rating and ineligibility for award. Bidders must carefully review and accurately complete all sections to demonstrate their capability and compliance with the solicitation requirements for debris removal services.

This document is the Contract Price Schedule for the "CAHA Debris Removal" project. It outlines the pricing structure for the base contract and any potential options.

• CLIN 10: Clean up existing debris as stated in the statement of services (Quantity: 1, Unit: Lump Sum).

• CLIN 20: Option 1 – 120 Tons of additional debris clean up (Quantity: 1, Unit: Lump Sum).

• CLIN 30: Option 2 – 60 Tons of additional debris clean up (Quantity: 1, Unit: Lump Sum).

• The schedule includes fields for Total Price for each CLIN and a Total Price for CLINs 10, 20, and 30.

• Offerors are required to submit pricing for both base line items and all option line items. Failure to do so may result in an unacceptable proposal.

• For lump-sum line items, the total price must be provided.

• Pricing will be evaluated based on best value to the Government.

• The Government reserves the right to award any or no options.

• The notice references FAR Clause 52.217-5, Evaluation of Options, and FAR 17.

Statement of Work (SOW) / Performance Work Statement (PWS)

To remove all collected housing debris from the Buxton Lifeguard Beach parking lot in Buxton, NC, and dispose of it properly, returning the parking lot to a clean asphalt state with a rehabilitated island.

• Removal of housing debris including wood, electrical wires, appliances, damaged hot tubs, furniture, and PVC pipes.

• Disposal of all debris in accordance with Federal, State, and local laws and ordinances.

• Minimizing the creation of deconstruction and demolition waste.

• Ensuring the underlying asphalt surface is not damaged during removal.

• Cleaning the parking lot area, including broom cleaning and magnetic sweeping of the asphalt.

• Filling any gouges or scrapes deeper than 3 inches with asphalt cold patch.

• Removing all apparatus, tools, equipment, and rubbish upon completion.

• Debris to be removed completely.

• Sand to be screened to ensure removal of small (6-10 inches) and medium (10-40 inches) sized debris pieces.

• Site to be graded level with surrounding grades.

• Asphalt parking lot to be broom cleaned and magnetically swept.

• Premises to be left clean, neat, and orderly.

• Commencement of work within seven (7) days of contract award.

• Completion of removal and cleanup within twenty-one (21) days of contract award.

• Operations limited to within the park property limits.

• A site visit is recommended prior to submitting a proposal.

• Contractor is responsible for verifying all measurements and site conditions before submitting a proposal.

• Contractor shall provide a Waste Management Plan.

• Contractor shall obtain COR approval for method and route of removal.

• Protective fencing, signing, and barricades must be approved by the COR.

• Hazardous materials must be disposed of according to NCDEQ and EPA regulations.

• Submittals required prior to mobilization include: Debris Removal Schedule, Accident Prevention Plan/Safety Plan/Site Plan, Disposal Plan (including Equipment Placement/Storage, Waste Management, and Routes of Travel), and List of Subcontractors (if applicable).

• Submittals required during debris removal include: Daily Reports, Waste Material Dump Receipts, Reuse/Salvage/Recycle Manifests/Invoices, and Schedule Updates or Changes.

• Safety plan must address Heavy Equipment Operations, Public/visitor Safety, Personal Protective Equipment (PPE), Emergency Response Protocols, and Personnel Training Requirements.

• Option 1: Removal of additional debris estimated at 120 Tons, if needed, within a 90-day window after contract award.

• Option 2: Removal of additional debris estimated at 60 Tons, if needed, within a 90-day window after contract award.

Wage Determination

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for services to remove debris, including wood, electrical wires, appliances, damaged hot tubs, furniture, and PVC pipes, from the Buxton Lifeguard Beach parking lot in Buxton, NC. The estimated debris amount is 720 tons.

• Removal of approximately 720 tons of debris from the Buxton Lifeguard Beach parking lot.

• Includes removal of wood, electrical wires, appliances, damaged hot tubs, furniture, and PVC pipes.

• Option for removal of additional debris (120 tons for Option 1, 60 tons for Option 2).

• Type of Contract: Firm Fixed Price.

• Period of Performance: 28 days from award for the base period (Item 00010). Options have different periods: Option 1 (Item 00020) and Option 2 (Item 00030) have anticipated exercise dates of 90 days after award, with performance periods of 90 days after award.

• Evaluation: The government intends to award to the responsible offeror whose proposal is the lowest price among those determined to be technically acceptable. Evaluation will be on a pass/fail basis for technical capability and price. Past performance will also be considered.

• Technical Submission: Must include Attachment 4 (LPTA Technical Acceptability Template) and address relevant experience, key subcontractors, SOW/specs compliance, and period of performance acknowledgment.

• Price Submission: Must include Attachment 3 (Price Schedule).

• This acquisition is a 100% small business set-aside procurement.

• Offerors must be registered in the System for Award Management (SAM) to be eligible for award.

• Funds are not presently available, but there is a high expectation of funds becoming available for award. No award will be made until funds are available.

• The contracting office will not provide hardcopies or email copies of the solicitation.

• No phone calls are permitted for inquiries.
