Key Requirements & Eligibility

- Submission of safety documentation demonstrating acceptable safety performance (Attachment G outlines ES&H requirements).
- Copies of all valid and current HVAC/chiller maintenance and repair licenses and permits.
- Compliance with Wage Determination (Illinois Kane SCA 2015-5741, Rev. 27).
- Adherence to specific insurance requirements (General Liability, Automobile Liability, Excess/Umbrella Liability, Workers' Compensation, Employer's Liability) as detailed in Attachment D.
- Completion of FFDG Proposal Certifications (Attachment F) and Subcontractor Annual Representations & Certifications (SARC) (Attachment E).
Evaluation
Award will be made using a Lowest Price Technically Acceptable (LPTA) source selection method. Technical and Past Performance factors will be evaluated on an acceptable/unacceptable basis. The award will go to the offeror submitting the lowest evaluated price whose proposal is technically acceptable.
