Scope of Work

Contractors will also facilitate Worker's Compensation claims (Form CA-16), provide health promotion and education, and offer limited CLIA-waived lab testing. Staffing requirements include 1 FTE Medical Provider, 1 FTE Occupational Health Nurse, .05 FTE Physician, and PRN support for 40 hours per week (Monday-Friday, 0800-1600). Additional services involve occupational surveillance (audiograms, blood lead level testing), routine and urgent infectious disease testing, and monthly health education messaging.
