The purpose of this IDIQ contract is to acquire professional occupational health services through a nationwide patient physician network (PPN) that owns a series of clinics across the country (in each state, territory, district, etc.) to acquire medical examinations and related services for prospective and current Federal Bureau of Investigation’s (FBI) employees in support of the FBI's global mission.  Services may be provided in fixed locations or via mobile services.  The Contractor shall adhere to both the RFP and Statement of Work.   

Update: Attachment E has been corrected

Update: The majority of questions have been answered and included in the attachment. The remaining highlighted questions will be answered the first week of January. 

Update: The due date will be updated to January 16th at Noon CT. The amended Attachment D has been included. The remaining ammendments will be added this week, along with the responses to the remaining questions.

Additional Question: Under Section 3.5.1, Objective E Task 1: Fitness for Duty, the Statement of Work states that “The Provider shall ensure that forms FD- 1065 and FD-1093 (or FD- 1093A) are completed in their entirety.”  While Attachment D, Pricing Spreadsheet_Amend 3,  does include OSHA Audiogram (Includes review of Hearing History) as part of the Fitness for Duty (Periodic) exam, the Statement of Work in this section does not include the requirement.  Please clarify whether audiogram will be required as part of all Fitness for Duty (Periodic) exams or as needed under the Audiogram Only line item?

Response - Applicant exams must have a pure tone audiogram completed for their pre-employment. On board employees with periodic FFD exam requirements receive an in-house audiogram done through OSEP's Shoebox program and not ordered for the vendor to complete. However, there are occasions when we will need to order a stand alone pure tone audiogram from the vendor d/t an inconclusive or "No Response" (NR) results. Please adjust proposals accordingly.
