Statement of Work (SOW) / Performance Work Statement (PWS)

This SOW outlines the requirements for maintaining roads, markings, and surfaces at Mountain Home Air Force Base, Idaho. The primary goal is to ensure proper road markings and pavement conditions.

• Road Marking Maintenance: Cleaning and restriping existing road markings, including centerlines, stop bars, and pedestrian crosswalks on various streets.

• Mill and Overlay: Performing milling and overlay operations on specified streets (1st Ave, 7th Ave, 12th Street, 14th Street, Ironwood Drive, and Trap St.) to a 2-inch depth. This includes applying new road markings as appropriate.

• Specific Streets: Work includes maintaining markings on Aardvark, Phantom, Alpine, Rosewood, Red Fir, and E Street, as well as mill and overlay on 1st Ave, 7th Ave, 12th Street, 14th Street, Ironwood Drive, and Trap St.

• Contractor must use their own survey data for construction drawings to maintain proper elevations and achieve adequate drainage.

• Mill and overlay work must match existing elevations, adjusted as needed for proper drainage (1% slope to edges).

• Contractor must provide expertise for proper drainage on new pavement.

• All work must meet OSHA standards and applicable safety laws.

• Nuclear density gauge authorization is required and may take up to 30 days.

• Contractor shall hold weekly coordination meetings with a 3-week lookahead.

• Location: Mountain Home Air Force Base, Idaho.

• Performance Period: 90 days.

• Safety: Compliance with OSHA, EM-385-1-1, and base safety procedures. Hard hats, safety shoes, and other PPE are mandatory. Heavy equipment requires backup alarms. Electrical safety compliance (NFPA 70 and 72E) is required.

• Traffic Control: A traffic control plan conforming to MUTCD is required. Flaggers must be used during milling operations.

• Submittals: Construction drawings, manufacturer's product data, and a submittal register are required. Government has 14 calendar days for review.

• Base Access: All workers must be vetted by the USG and obtain base access via SF Form 30. Adherence to base entry protocols is mandatory.

• Personnel: Qualified personnel are required, including a Superintendent (min. 10 years experience), Quality Control Manager (min. 5 years experience), and Site Safety and Health Officer (SSHO) meeting EM-385-1-1 requirements.

• Environmental: Soil erosion and sediment controls must be implemented per Specification 01 57 19.

• Warranty: One (1) year from final acceptance, with specific response times for emergency (4-hour) and routine (3-day) warranty calls.

• Contractor Facilities: Laydown lots and temporary facilities must be approved and restored to original condition. Fencing and screening are required for material storage yards.
