This is a request for information and will be used for market research purposes only. The sources sought notice is used to identify potential vendors that have the resources and capabilities to provide the TV/Cable/Telecommunications services to the 321 units in the Washington DC Veteran Affairs Medical Centers:

SCOPE OF WORK:

The contractor shall be required to furnish all necessary and appropriate labor, supervision, materials, equipment and supplies to provide CABLE/TV in accordance with the terms and conditions of the resulting contract and the specifications and requirements, as specified herein. The contractor shall provide all necessary cable boxes for 321 units within the Medical Center, Community Living Center (CLC), and Fisher House.

EQUIPMENT:

In excess of 321 TV boxes, etc., for the Main Hospital, CLC, and Fisher House. The contractor shall provide all tools and equipment necessary to properly and safely perform the services set forth within this Statement of Work (SOW). This shall include, but is not limited, to all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner.

TASK FREQUENCY AND INSTRUCTIONS:

Contractor shall provide all the necessary equipment to connect the hospital TV s with an appropriate cable system to provide extended cable such as Classic HBO, STARZ, Super Pack, Sonic Entertainment, Extra Packs, etc.. The contractor shall provide maintenance of the boxes as needed for all 321 units.

PROPERTY DAMAGE:

The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damage immediately and shall be assessed current replacement costs for property damaged by thew contractor, unless corrective action is taken. Any damaged material, (i.e., trees, shrubs, lawn/turf, curbs, gutters, sidewalks, etc..) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the government, upon approval of the Contracting Officer or Contracting Officer Representative.

INTERESTED & CAPABLE RESPONSES

NCO 5 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources and capabilities necessary to provide these services. Please respond with your:

1. Business name, SAM Unique Entity Identifier (UEI), business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), and person of contact (including telephone number & email address).

2. GSA contract number; if you have one,

3. Capability statement and summary of relevant experience conducting TV/Cable telecommunications services for Veterans Health Administration (VHA) and other healthcare entities.

4. Approximate cost estimate to perform the work described per year, assuming all 321 units require a TV box and subscription. Likewise, if you believe the Price/Cost Schedule can be improved for applicability, you are encouraged to suggest a CLIN structure that should be used in a future contract. Your estimate shall not be considered a final quote, rather, the estimate shall be used to assist contracting with determining fair and reasonable pricing when drafting the Governments independent estimate.
