This addendum revises FAR 52.212-1 Instructions to Offerors—Commercial Products and Commercial Services.

• Period of Acceptance of Offers: Revised to 120 days.

• Debriefing: Revised to provide explanations for unsuccessful quoters when using simplified procedures at 12.201-1. If an award notice was not required to be posted to the GPE, information that would be included in an award notice (per 5.301(c)) must also be provided.

Statement of Work (SOW) / Performance Work Statement (PWS)

To ensure the safety and sanitary operation of the VA Central California Health Care System (VACCHCS) cooking facilities by providing professional cleaning and maintenance of the Canteen & Patient Kitchen (Nutrition and Food Service - NFS) hood exhaust systems and grease traps.

• Provide comprehensive cleaning, inspection, and maintenance of specified hood exhaust systems and grease traps.

• Services include cleaning hoods, ductwork, fans, and grease traps to bare metal or to remove all fats, oils, and grease (FOG).

• Replace filters that cannot be satisfactorily cleaned.

• Comply with NFPA 96 (2024 Current Edition) and all other applicable federal, state, and local standards.

• Document and report any defects found in the systems that require repair.

• Frequency:

  • West Canteen Exhaust Hood System (Hood #2): Monthly
  • East Canteen Exhaust Hood System (Hood #1): Quarterly
  • Canteen Grease Traps: Quarterly inspection and cleaning
  • NFS Hood Exhaust Systems: Quarterly

• Cleaning to be performed after work hours (Canteen ends 5 pm, NFS ends 7 pm), with at least 24 hours' notice for schedule changes.

• Submit before & after photos and a full maintenance/cleaning report within 5 business days of each service visit.

• Affix a Certificate of Performance label to each hood after cleaning.

• Report any deficiencies within 2 business days of identification.

• Services will be evaluated based on accuracy, completeness, timeliness of reports, and quality of work, with specific performance thresholds outlined in the Quality Assurance Surveillance Plan (QASP).

• Contractor personnel cannot work alone in interstitial spaces or on rooftops; minimum team of two required, with COR or POC presence.

• Contractor must check in with COR/POC before and after work.
