Statement of Work (SOW)

To provide safety shoes to employees at the DLA Distribution Center San Joaquin, CA, via a mobile shoe unit.

• A safety shoe mobile will visit the DLA Distribution Center San Joaquin, CA, once a month.

• Visits will occur during the hours of 0900 to 1630 (9:00 A.M. – 4:30 P.M.).

• The government allowance is $165.00 per employee per year for regular safety shoes and metatarsal guard shoes.

• Employees must present a completed and signed DLA DISTRIBUTION J7 / FORM 1019 to obtain shoes.

• The contractor must provide an electronic copy (PDF) of all completed Form 1019s to the Government Point of Contact (POC) within five days of service.

• Invoices will be submitted for the actual price of footwear up to the government subsidy maximum. Any amount above the subsidy, or for comfort items like insoles and shoelaces, is the employee's responsibility.

• Approximately 800 employees per year qualify for safety shoes.

• The contract period of performance is five years, from April 26, 2026, to April 25, 2031.

• The service location is DLA Distribution Center San Joaquin, CA, WHSE 16.

• Contractors must comply with Defense Logistics Agency (DLA) safety and health regulations.

• Mishaps involving contractor employees must be reported immediately.

• Contractors must accept payment in Wide Area Work Flow.

• Personnel performing work must comply with facility security regulations, including traffic laws, vehicle registration, and personnel entrance requirements.

• All personnel requiring access must possess a valid state driver's license, vehicle registration, and proof of insurance.

• Contractor personnel will be issued visiting passes/badges.
