Foreign vendors are required to complete AMS-Form-050 (Enclosure B) and Form W-8 (Enclosure F). U.S. vendors must provide Form W-9 (Enclosure E). Selected vendors will also need to complete a Vendor ACH Authorization Form (Enclosure D).
- Evaluation: Award will be made to the responsive and responsible offeror submitting the lowest total price who meets all technical requirements. Offerors taking exceptions to BSA terms and conditions or failing to provide all requirements may be deemed non-responsive. Alternate products must demonstrate equivalent or greater performance with sufficient documentation.
