This is a combined synopsis/solicitation for commercial items, issued as a Request for Quotation (RFQ), to procure on-site Self-Contained Breathing Apparatus (SCBA) Physicals and SCBA Fit Testing, and Spirometry testing for employees of the Six Rivers National Forest. This acquisition is a Total Small Business Set-Aside.

• Annual on-site SCBA Physicals, SCBA Fit Testing, and Spirometry Testing.

• Provide documentation of tests taken to government employees at the time of service.

• Provide an itemized cost and accounting summary to the Contracting Officer (CO) / Contracting Officer's Representative (COR) for each day tests are given.

• Qualified licensed Physician or Other Licensed Health Care Professional (PLHCP) to perform required medical tests.

• Contract Type: Firm-Fixed Award anticipated.

• Period of Performance: April 6, 2026 – September 1, 2026.

Statement of Work (SOW)
