Statement of Work (SOW)

To establish requirements for the procurement, delivery, installation, training, and ongoing support of Hematology analyzers and associated equipment for diagnostic testing at Great Plains Area Indian Health Service (GPA IHS) healthcare facilities.

• Provide new hematology analyzers and all associated equipment, consumables, installation, validation, training, and warranty support.

• Deliverables include analyzers and peripherals, initial reagent supply, installation, calibration, verification, user training, LIS integration (if required), and warranty/service coverage.

• Equipment must meet CLIA standards and FDA device requirements.

• Performance must meet CLIA and manufacturer standards.

• Preventive maintenance to be performed at least annually.

• Onsite repair response within 48 hours for hospitals and next business day for clinics.

• Remote support available 24/7.

• Contract period is Base plus 4 (B+4) from the date of award.

• Covers procurement, delivery, installation, training, validation, and warranty services.

• Facilities are located across South Dakota, North Dakota, and Montana.

• Analyzers must be automated CBC with 6-part differential, capable of handling projected test volumes, and interface with LIS.

• Must meet HHS/IHS cybersecurity standards.

• Must conform to Revised Section 508 Standards and WCAG 2.0 Level AA.

• Pricing based on a firm fixed Cost-Per-Test (CPT) model, including all equipment, reagents, consumables, maintenance, and training.

Sources Sought Notice

This notice is a market research tool to determine the availability of qualified Indian Small Businesses Economic Enterprises (ISBEEs) and Indian Economic Enterprises (IEEs) capable of procuring, delivering, installing, training, and supporting hematology analyzers and associated equipment for the Great Plains Area Indian Health Service (GPA IHS). This is NOT a solicitation or request for proposal/quote, and no contract will be awarded from this notice.

• Continuous automated/real-time calibration and verification.

• Contract Type: Firm-Fixed-Price, Monthly Flat Rate, Indefinite Delivery Indefinite Quantity (ID/IQ).

This document is the Indian Economic Enterprise (IEE) Representation Form, required under the Buy Indian Act. It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as defined by HHSAR 326.601. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Eligibility Period: The enterprise must meet the IEE definition at the time of offer, contract award, and throughout the contract performance period. Any change in eligibility requires immediate notification to the Contracting Officer.
• Documentation: Contracting Officers may require documentation of eligibility, and awards may be subject to protest if eligibility is questioned.
• Registration: Successful Offerors must be registered with the System of Award Management (SAM).
• Penalties: False or misleading information is a violation of law, punishable under 18 U.S.C. 1001, and false claims are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
• Representation Section: Offerors must indicate whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, DUNS Number, Certifying Signature, and Print Name.
