Sources Sought Notice

This notice is a request for capability information for the provision of an OrthoScan TAU 2020 Mini C-Arm, accessories, and extended warranties to the Indian Health Service (IHS), Phoenix Indian Medical Center in Phoenix, AZ. This is NOT a solicitation for proposals, and no contract will be awarded from this notice. The government is conducting market research to identify interested and capable parties.

• 1x OrthoScan TAU 2020 Mini C-Arm (Item #1000-0017)
• Various accessories (Digital Printer, Wireless Foot Switch, Touchscreen OrthoTouch 2.0, LCD Monitor, IDR Package, Wireless Network Card)
• 1 Year Customer Warranty (all parts and labor)
• 4 Years Additional Platinum Elite Service Contract
• Installation, Configuration, and Shipping

Required submission elements include:

1. Company name
2. Point of contact information
3. FSS / SEWP Contract number (if applicable)
4. DUNS or SAM Unique Entity Identifier
5. Tax ID number
6. Indication of business category (IEE, ISBEE, SB, etc.)
7. Capability statement highlighting past performance (within 5 years) for similar deliveries.
8. Documentation of authorization as a distributor from the manufacturer.
9. IHS Indian Economic Enterprise Representation form (if applicable) and identification of the Tribe/Corporation for IEE status.

This requirement may be set-aside for Indian Small Business Economic Enterprises (ISBEE) or Small Businesses (SB). Priority will be given to ISBEE set-asides. The government will evaluate market capacity to determine the acquisition strategy. Definitions for Indian Economic Enterprise (IEE) and ISBEE are provided.

• Firms must be registered in SAM and have completed electronic annual representations and certifications.

• No feedback or evaluations will be provided on submissions.
