• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, Certifying Signature, SAM UEI Number, and Print Name.

Statement of Work (SOW) / Performance Work Statement (PWS)

To conduct annual load bank testing and fuel quality testing of Emergency Power Supply Systems (EPSS) at various facilities within the Great Plains Area Indian Health Service (IHS) to maintain accreditation from the Centers for Medicare and Medicaid Services and the Joint Commission.

• Perform annual load bank and fuel quality tests on emergency generators at specified Great Plains Area IHS facilities.

• Provide all necessary material, labor, equipment, and supervision.

• Load bank testing includes pre-startup inspection, generator startup, load transfer (annual and 3-year/4-hour tests), and post-test procedures.

• Fuel quality testing involves obtaining individual fuel samples from each generator fuel tank and performing tests using appropriate ASTM standards.

• The contract period includes a Base Year (Date of Award – 12/31/2026) and four Option Years (through 12/31/2030).

• Locations include facilities in North Dakota and South Dakota: Belcourt, ND; Eagle Butte, SD; Ft. Thompson, SD; Ft. Yates, ND; Kyle, SD; Lower Brule, SD; Mobridge, SD; McLaughlin, SD; Pine Ridge, SD; Rapid City, SD; Rosebud, SD; Sisseton, SD; Wagner, SD; and Wanblee, SD.

• Testing and inspection hours are 8:00 AM to 5:00 PM, Monday-Friday, unless otherwise approved.

• Load bank tests require specific load percentages (at least 50% and 75% for annual; at least 30% and 75% for 4-hour) for defined durations.

• Fuel quality tests must adhere to appropriate ASTM standards.

• Reports must be completed and submitted to the Project Officer within 20 workdays of the final site visit.

• Contractor personnel must possess credentials from relevant organizations (e.g., NFPA, EGSA, Licensed Electrician).

• Contractor must comply with facility manager requirements and clean up after services.

• Failed component replacement and repairs are not included unless caused by contractor activities; recommendations for repairs may be provided.

• Invoices must be submitted through the Invoice Processing Platform (IPP).

• A post-award meeting will be conducted to establish schedules and provide contact information.
