Submission Requirements

- Ability to provide services at the Blackfeet Service Unit location.

- A list of past jobs/contracts within the last five years providing similar biomedical services.
- A completed Buy Indian Act Indian Economic Enterprise Representation Form (attached as "B11 - IEE Representation.pdf"). This form is a critical self-certification for IEE eligibility, requiring confirmation of 51% Indian/Tribal/ANC ownership, 51% earnings to them, and Indian control of management and daily operations. Failure to meet IEE definition or provide accurate information can lead to disqualification or penalties.
