Statement of Work (SOW) / Performance Work Statement (PWS)

To provide non-personal Professional Medical Laboratory Technician (MLT) Services to the Phoenix Indian Medical Center (PIMC) in Phoenix, Arizona, on an "as needed" basis to cover recurring needs due to staff changes and shortages.

• Provide qualified and experienced MLTs to perform laboratory functions including specimen collection, processing, analysis, and verification of results.

• Services encompass major areas of clinical laboratory work: clinical chemistry, hematology, coagulation, blood bank, serology, urinalysis, microbiology, and molecular testing.

• MLTs must adhere to IHS facility infection control and safety standards, as well as standards from CAP, CMS, and The Joint Commission (TJC).

• Employees must be able to meet physical demands, work tactfully in stressful situations, and interact with diverse populations.

• Adherence to all IHS, local, federal, and state regulatory requirements.

• Compliance with CAP, CMS, and TJC standards.

• Consistent performance of patient care processes.

• Maintenance of acceptable personal hygiene and grooming standards.

• Quality control program to identify deficiencies and ensure appropriate services.

• Performance reviewed for conformance to IHS policy, procedures, and accepted professional practices.

• Period of Performance: 12-month base period and four (4) 12-month option periods.

• Location: Phoenix Indian Medical Center (PIMC), 4212 N. 16th Street, Phoenix, AZ 85016.

• Security Clearance: Full IHS mandated security pre-clearance, including fingerprint checks and OIG exclusion list checks, is required prior to service commencement.

• Candidate Notification: Provide and identify candidates at least eight (8) weeks in advance of the scheduled performance date due to clearance and credentialing timelines.

• Qualifications: Minimum of one (1) year experience as an MLT. Education requirements include an Associate's degree in Biology or Chemistry with specific program completion or equivalent experience. Certification/licensure required (ASCP or AMT, BLS/ACLS).

• Orientation: Mandatory facility and department-specific orientation, security clearance, and PIV enrollment prior to commencing duties.

• Reporting: Submit employee profiles to COR prior to service. Timesheets submitted to supervisor for accuracy and certification. Invoices submitted electronically via IPP.

• Compliance: Adherence to Pro-Children Act of 1994, Indian Child Protection and Family Violence Prevention Act, HHS Tobacco-Free Policy, and Crime Control Act of 1990.

• Shift Coverage: Minimum fifteen (15) calendar days' notice for scheduled coverage. 72-hour notification required for PRN/Replacement Contractor employee(s) for designated shifts. Emergency situations require good-faith effort to accommodate requests with less than 15 days' notice.
