Statement of Work (SOW) / Performance Work Statement (PWS)

To acquire non-personal professional Inpatient and Outpatient Physician services for the Colorado River Service Unit (CRSU), specifically at the Parker Indian Health Center in Parker, Arizona. The goal is to minimize disruptions in clinical services due to staff shortages and to cover consistently changing needs.

• Provide non-personal professional Physician services for a 12-month base period with four (4) one-year option periods.

• Services are to be provided on an "as-needed" basis.

• Cover Inpatient Department services 24 hours a day, 7 days a week.

• Cover Outpatient Department services Monday through Friday, 7:30 am to 6:00 pm (excluding federal holidays).

• Specific days and shift times are subject to change based on medical needs.

• Services include general medical, ambulatory, emergency medicine, pediatrics, community health, preventative health, and mental health.

• Physicians will be expected to have the necessary education, training, licensing, and experience.

• Work performance must be consistent with facility guidelines and procedures, including TJC accreditation standards.

• Chart notes are to be completed on the day services are rendered.

• Utilize the electronic health record for chart documentation.

• Be sensitive to unique cultural factors of patients, including use of interpreters if necessary.

• May be required to work beyond normal duty hours, on weekends, holidays, and after regularly scheduled duty hours on an as-needed basis for emergencies or inpatient coverage.

• Performance will be evaluated based on performance standards within the SOW.

• Contractor must maintain acceptable standards of personal hygiene, grooming, and professional attire.

• Quality Assurance Surveillance Plan (QASP) will be used to monitor contractor performance.

• Location: Parker Indian Health Center, 12033 Agency Road, Parker, Arizona 85344.

• Period of Performance: 12-month base period with four (4) one-year option periods.

• Qualifications: Board certification or eligibility in Family Medicine, Internal Medicine, or Pediatrics. Applicants require at least three (3) years of current experience. Must hold an unrestricted license to practice in the U.S. Current BLS and ACLS certifications required.

• Security Clearance: Candidates must undergo Indian Health Service (IHS) mandated security clearance, including fingerprint and background checks. This process may take six (6) weeks or more.

• Credentialing: Submission of various forms including resume, OF-306, IHS Provisional Access Memo, and other required certifications.

• Accommodations/Lodging: Contractor shall provide accommodations or housing allowance. Housing availability in Parker is limited.

• Reporting: Timesheets to be submitted to the Deputy Clinical Director or Clinical Director. Invoices to be submitted electronically via US Treasury in IPP.

• Compliance: Adherence to Pro-Children Act of 1994, Indian Child Protection and Family Violence Prevention Act, HHS Tobacco Policy, and Crime Control Act of 1990.

• Personnel Replacement: Contractor shall relieve contracted staff within 24 hours of notice, with replacement available within one week. Grounds for removal/replacement may include services not performed in accordance with contract terms.

This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to declare whether they meet the definition of an "Indian Economic Enterprise" (IEE) as defined by the Buy Indian Act. This form is required in response to Sources Sought Notices, Requests for Information (RFIs), or Solicitations.

• Representation: Offerors must indicate whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Eligibility Requirements: The enterprise must meet the IEE definition at the time of offer, contract award, and throughout the contract performance period. Any change in eligibility requires immediate notification to the Contracting Officer.
• Required Information: The form requires the name of the 51% owner of the IEE firm, the name of the Federally Recognized Tribal Entity (if applicable), the Business Name, the Certifying Signature, the printed name of the certifier, and the Unique Entity Identifier (UEI) Number.
• Legal Consequences: False or misleading information is a violation of law and punishable under 18 U.S.C. 1001, with false claims subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.

This form is critical for bidders responding to opportunities that are set aside or sole-sourced under the Buy Indian Act. Bidders must accurately complete this form to self-certify their eligibility as an Indian Economic Enterprise. Failure to meet the definition or accurately represent the enterprise's status can lead to disqualification or penalties. Offerors may also be required to provide documentation of eligibility and must be registered in the System of Award Management (SAM).
