Statement of Work (SOW) / Performance Work Statement (PWS)

This document outlines the requirements for repairing a collapsed sewer drain line in the men's and women's restrooms at Building 888, Gunter Annex, Montgomery, AL. The specific issue is a collapsed drain line behind four toilets on the second floor of the women's restrooms.

• Locate and Repair/Replace Drain Line: Identify the cause of the blockage (toilets not draining) and the location of the collapsed or damaged pipe, potentially using a borescope. Repair or replace the affected sewer drain pipe.

• Wall Repair: Repair the drywall after accessing the pipe.

• Finishing: Caulk and re-tile or paint the repaired wall.

• Compliance: All work must adhere to state, federal codes, and OSHA standards.

• Restoration: Restore any disturbed areas to their original condition, including turf, tire marks/ruts, and damaged tile or concrete cart paths/sidewalks.

• Workplace Safety: Maintain a clear, hazard-free, and safe workplace, ensuring the site is secured at the end of each day.

• Clean Up: Clean the work site daily, including construction areas, office areas, material storage, parking, and eating areas.

• Equipment Storage: Overnight trailers/equipment must be stored in an approved location with visible identification.

• Warranties: Furnish manufacturer warranties for all incorporated components and equipment.

• Duration: The contractor shall have 60 calendar days to complete the work.

• Work Hours: Work will generally be performed during normal working hours (6:00 AM to 5:00 PM). Work on weekends or federal holidays requires prior coordination with the Technical Monitor/Contract Administrator at least 72 hours in advance.

• Building Occupancy: The building will be occupied during construction.

• Site Visit: Contractors are responsible for conducting a site investigation.

• Drawings and Specifications: Must comply with contract drawings and specifications.

• Submittals: Requires submission of AF Form 3000, Material Submittal Approval, including manufacturer's descriptive data, certifications of conformance, and potentially samples. Shop drawings and samples also need to be submitted.

• Environmental Protection: Adherence to environmental protection requirements, including handling of hazardous materials, stormwater BMPs, and protection of existing landscape. No friable asbestos-containing materials are to be installed.

• Radioactive Materials: Specific procedures apply if radioactive materials are to be used on site, requiring written approval from the Base Radiation Safety Officer.

• Asbestos/Lead-Based Paint: Compliance with OSHA regulations and state certification is required if abatement work is involved.

• Hazardous Materials: All hazardous chemicals used must be registered with the Maxwell AFB Hazardous Material Manager.

• Excavation: Requires an approved Construction Permit (AF Form 103) before any excavation.

• Ground Fault Circuit Interrupters (GFCI): Required for portable electrical tools in outside or wet locations.

• Equipment Manuals: Submission of three copies of equipment operating, maintenance, and repair manuals is required.

• Traffic Control: Contractor is responsible for orderly traffic handling.

• Barricades: Must furnish, place, and maintain required barricades.

• Final Cleaning: Execute final cleaning prior to final inspection.

• Security: Compliance with security requirements for unclassified contracts, including notification, employee listings, passes, identification, key control, and reporting threats to security. Possession of weapons, firearms, or ammunition is prohibited.

• Insurance: Required insurance includes Workmen's Compensation, General Liability, and Automobile Liability with specified minimum limits.

• Inspection and Acceptance: 42 CES/CEO TM is responsible for inspection; CO is responsible for final acceptance. The TM POC is David Ingram.

This document is a Sources Sought notice issued by the 42d Contracting Squadron at Maxwell Air Force Base, Alabama. It seeks to identify potential sources capable of providing Boiler Replacement and Services for "JUBJ 26-8336 G888 Restroom Repairs." This is for information and planning purposes only and is not a solicitation or a request for SF330s.
