Statement of Work (SOW) / Performance Work Statement (PWS)

To provide emergency residential treatment, rehabilitative, and supportive services to homeless geriatric Veterans (age 50+, with exceptions considered) as part of the Department of Veterans Affairs Palo Alto Health Care System's (VAPAHCS) Community Based Health Care for Homeless Veterans (HCHV) program. The goal is to offer a safe, community-based residential environment to improve housing situations and facilitate recovery.

• Residential Room and Board: Provide single rooms with furnishings, 24/7 access, and storage for personal belongings.

• Dietetic Services: Provide three nutritious meals daily, plus snacks, accommodating dietary restrictions and cultural/religious preferences. Facilities must maintain sanitary conditions and offer pantry access.

• Laundry Facilities: Provide free laundry facilities and detergent, with assistance available.

• Case Management Services: Maintain a ratio of one dedicated, full-time case manager per 12 Veterans. Services include intake, psychosocial assessments, individualized care plans, crisis prevention, individual counseling, financial planning, housing search assistance, and coordination with VA staff and external providers. Specific requirements for High Suicide Risk (HSR) Veterans and medication management are detailed.

• Permanent Housing Search: Assist Veterans in transitioning to permanent housing, including assessments, developing housing plans, and accessing resources.

• Financial Planning and Employment: Assist Veterans in accessing financial resources (SSI, SSDI, etc.) and provide employment support and referrals.

• Group Activities: Offer structured group activities (e.g., therapy, life skills) twice monthly.

• Transportation: Assist Veterans with transportation to scheduled meetings, medical appointments, and hospital visits.

• Outreach: Engage in community outreach at least twice monthly.

• Documentation: Maintain individual case records, including assessments, progress reports, and discharge summaries, in compliance with privacy regulations.

• Maintain a minimum of 85% occupancy rate, with a preferred rate of 90%.

• Meet Total Quality Improvement (TQI) measures, including transition to independent housing (55%) and discharge with negative exits (20%).

• Respond to VA inquiries and referrals within 4 hours during business hours.

• Make admission determinations within 24 hours of referral.

• Report critical incidents within 24 hours.

• Services to be provided on-site at the Contractor's facility.

• Initial stay expected to be no longer than 60 days, with extensions possible under specific conditions.

• Contractor staff must be qualified professionals.

• Facilities must meet fire safety, sanitation, and accessibility standards (e.g., ADA accessible rooms and restrooms).

• A dedicated, licensed medical case manager is required.

• Staff must be trained in emergency protocols, CPR, First Aid, and de-escalation techniques.

• Adherence to COVID-19 requirements, including PPE provision and cleaning protocols.

• Services must be provided in a manner free from religious discrimination.

• Reasonable accommodations for disabilities must be provided.

The Department of Veterans Affairs (VA) Network Contracting Office (NCO 21) is conducting market research to identify potential vendors capable of providing Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) for the Veteran Affairs Palo Alto Health Care System (VAPAHCS) in Monterey and Santa Cruz Counties. A draft Performance Work Statement (PWS) is attached.
