This notice is for market research purposes only to identify potential sources for annual preventative maintenance of Gurneys & Power Loaders at the Indian Health Service, Chinle Comprehensive Health Care Facility. This is NOT a solicitation.

• Annual preventative maintenance for Gurneys & Power Loaders.

• Coverage includes parts, labor, travel, annual preventative maintenance, and battery coverage.

• The contractor must be certified to service Stryker model 6390 Power load cot fastener systems and Stryker Power-Pro XT cots.

• Period of Performance: One one-year Base Period with two one-year Option Periods.

• NAICS Code: 339113 Surgical Appliance and Supplies Manufacturing.

• Small Business Size Standard: 800 employees.

• Interested parties must submit a response by March 18, 2026, at 5:00 PM MST.

• Submissions should be sent to Teola Autaubo at teola.autaubo@ihs.gov with the subject line "SSN IHS1521098".

• Required information includes:

  • Company Profile (UEI, Name, Address, POC, business size/ownership, socio-economic status, Native American ownership if applicable, SAM.gov representations).
  • Brief statement of capability related to the requirement, including past performance and established contracts.
  • Completed IHS IEE Representation Form if claiming Indian-owned status.

• Telephone inquiries will not be accepted.

• Companies must be registered in SAM.gov.

• The "Indian Economic Enterprise Representation Form" is provided for entities claiming Indian-owned status.

• The government is determining acquisition strategy, including competition and set-asides.

• Submission is voluntary, and the government assumes no financial responsibility for costs incurred.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide a service contract for cots and power loaders, including parts, labor, travel, annual preventive maintenance, and battery coverage.

• Service three Stryker model 6390 Power load cot fastener systems and three Stryker Power-Pro XT cots.

• These cots and power loaders are used for loading and moving patients into ambulances.

• The power loaders have the capability to lift up to 750 pounds.

• The contractor must be certified to service the specific Stryker gurneys and power loaders owned by the hospital.

• Service to be performed at Chinle Comprehensive Health Care Facility, U.S. Hwy 191 and IR 102, Chinle, AZ, 86503.

• The service agreement expired in August of 2025.

• Patient transports were 1,125 in 2023, 1,473 in 2024, and 1,225 year-to-date in 2025.

• The equipment is essential for moving patients safely and protecting staff from injuries.

• The service has been performed on-site multiple times over the past three years.

• Coverage includes parts, labor, travel, annual preventive maintenance, and battery coverage.

• Contractor must be certified to service the specified equipment.

This document is the Indian Health Service (IHS) Buy Indian Act Indian Economic Enterprise (IEE) Representation Form. It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as defined by HHSAR 326.601, in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Eligibility Requirements: The enterprise must meet the IEE definition at the time of offer, contract award, and throughout the contract performance period. Contractors must notify the Contracting Officer immediately if they no longer meet these requirements.
• Documentation: Contracting Officers may require eligibility documentation before awarding Buy Indian Act contracts, and awards are subject to protest if eligibility is questioned.
• Registration: Successful Offerors must be registered with the System of Award Management (SAM).
• Penalties: False or misleading information is a violation of law, punishable under 18 U.S.C. 1001, and false claims are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
• Representation: Offerors must indicate whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, DUNS Number, Certifying Signature, and Print Name.

This form is critical for any bidder seeking to be considered for opportunities under the Buy Indian Act. Bidders must accurately complete this self-certification to demonstrate their eligibility. Failure to meet the IEE definition or provide accurate information can lead to disqualification or penalties.
