• Penalties: False or misleading information is subject to legal penalties.
• Representation Section: Requires the offeror to indicate if they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Information Fields: Includes spaces for "Name of Federally Recognized Tribal Entity," "Name of the 51% owner of IEE Firm," "Business Name," "DUNS Number," "Certifying Signature," and "Print Name."

This form is critical for bidders who intend to claim status as an "Indian Economic Enterprise" to be eligible for certain contract set-asides or sole-source awards under the Buy Indian Act. Bidders must accurately complete this form and ensure they meet the defined criteria throughout the contract lifecycle. Failure to meet these requirements or provide accurate information can lead to penalties and ineligibility.

Sources Sought Notice / Market Research

This notice is a request for capability information to identify potential sources for Antivenin, Centuronides Immune FAB2, Anascorp Injection for the Indian Health Service (IHS), Phoenix Area Office (PAO). This is NOT a solicitation for proposals, and no contract will be awarded from this notice. The government is conducting market research to determine potential market capacity and acquisition strategies.

• The IHS Contracting Officer will give priority to ISBEE set-asides.

• Acquisitions may be set-aside exclusively for ISBEEs, Small Businesses, or procured through full and open competition based on responses.

• Definitions for Indian Economic Enterprise (IEE) and Indian Small Business Economic Enterprise (ISBEE) are provided.

1. Company name
2. Point of contact information
3. FSS / SEWP Contract number (if applicable)
4. DUNS or SAM Unique Entity Identifier
5. Tax ID number
6. Indication of business category (IEE or ISBEE)
7. Capability Statement highlighting past performance (within 5 years) for similar deliveries.
8. Documentation from the manufacturer proving authorization as a distributor.
9. Representation on the designated IHS Indian Economic Enterprise Representation form (attached).
10. Identification of the Federally Recognized Indian Tribe or Alaska Native Corporation for IEE status.
