This notice is for market research purposes only, conducted by the Indian Health Service, Chinle Service Unit. The government is seeking information to identify potential sources for a Blanket Purchase Agreement (BPA) for Disposable Trocar Instruments required for daily surgeries. This is NOT a solicitation.

The government is interested in companies capable of providing disposable trocar instruments. A list of specific instruments and their specifications is provided in the document, including various sizes and types such as optical access systems, balloon blunt tips, shield bladed access systems, retrieval systems, and surgical scissors.

• Period of Performance: Base Year with Four (4) Year Options.

• Interested parties must submit a response by March 14, 2026, 5:00 PM MST.

• Responses are voluntary and the government assumes no financial responsibility for costs incurred.

• Telephone inquiries will not be accepted.

• Companies must be registered in the System for Award Management (SAM.gov).

• Submissions must include a Company Profile (UEI, Name, Address, POC, business size, ownership type, socio-economic status like 8(a), WOSB, SDVOSB, HUBZone, Native American ownership if applicable, SAM.gov representations/certifications).

• A brief statement of capability related to the requirement, including past performance and established contracts (GSA/VA Schedule), is required.

• If claiming Indian-owned Small Business Economic Enterprise (ISBEE) or Indian-owned Economic Enterprise (IEE), the IHS IEE Representation Form must be submitted.

• The applicable NAICS code is 339113 (Surgical Appliance and Supplies Manufacturing) with a small business size standard of 800 employees.

• Information collected will be used to determine acquisition strategy, including competition and set-asides.

• Registration: Successful Offerors must be registered with the System of Award Management (SAM).
• Penalties: False or misleading information is a violation of law, punishable under 18 U.S.C. 1001, and false claims are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
