• A wide range of respiratory therapy supplies, including sterile water, BiPAP circuits, oscillatory PEP systems, HMEF filters, ventilator circuits, suction systems, various mask types (AF541), air regulators, and nebulizers.

• Supplies must be compatible with the HVT (High Velocity Therapy) 2.0 System.

• All items must be packaged in sterile bags/containers, disposable, and suitable for direct patient care.

• Vendors must provide government pricing or better.

• Vendors must have an active Unique Entity Identifier (UEI) registered with SAM.

• Quotes must include an itemized list of supplies, quantity, and cost.

• Orders must be processed within 48 hours.

• Invoices must be submitted through the Invoice Process Platform (IPP).

• Vendors must provide technical expertise regarding respiratory products.

This document is the Indian Health Service (IHS) "Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as defined by the Buy Indian Act and HHSAR 326.601. This form is required in response to Sources Sought Notices, Requests for Information (RFIs), or Solicitations where a contract will be awarded under the Buy Indian Act.

• Self-Certification: Offerors must certify whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Eligibility Period: The enterprise must meet the definition throughout the offer period, at the time of contract award, and during the full contract performance period. Any change in eligibility must be immediately reported.
• Required Information: The form requires the Offeror to provide:
  • Name of Federally Recognized Tribal Entity (if applicable)
  • Name of the 51% owner of the IEE Firm
  • Business Name
  • DUNS Number
  • Certifying Signature and Print Name
• Legal Consequences: False or misleading information is punishable under federal law (18 U.S.C. 1001, 31 U.S.C. 3729-3731, 18 U.S.C. 287).
• Additional Requirements: Successful offerors must also be registered with the System of Award Management (SAM).

Bidders responding to opportunities that invoke the Buy Indian Act must complete and submit this form to self-certify their status as an Indian Economic Enterprise. Failure to meet the eligibility requirements or accurately complete this form may affect a bid's eligibility for award or lead to penalties. Contracting Officers may require further documentation to verify eligibility.
