• Business Activity Description: A detailed description of the entity's relevant business activities.
• Certifications: Statements regarding citizenship/residency status, the need for the data, agreement to comply with export control laws, and assurances that no personnel are debarred, suspended, or have violated export control laws.
• Contractor Certification: Signature and date from the authorized representative.
• JCO Action: Section for the U.S./Canada Joint Certification Office (JCO) to record acceptance, certification number, and expiration date.

Bidders who are required to access or handle militarily critical technical data as part of an opportunity must complete and submit this form. The form ensures compliance with export control regulations and verifies the entity's eligibility to receive such data. Failure to have an accepted DD Form 2345 may prevent a bidder from obtaining necessary technical information to prepare a compliant proposal or perform the contract.
