• Self-Certification: Offerors must certify whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act.
• Eligibility Period: The enterprise must meet the definition throughout the offer period, at the time of contract award, and during the full contract performance period.
• Notification Requirement: Contractors must immediately notify the Contracting Officer if they no longer meet eligibility requirements.
• Documentation: Contracting Officers may require documentation of eligibility.
• Registration: Successful offerors must be registered with the System of Award Management (SAM).
• Penalties: False or misleading information is subject to legal penalties.

Statement of Work (SOW) / Performance Work Statement (PWS)

To procure a diagnostic testing instrument for arterial, venous, and pleural blood gas analysis for the Phoenix Indian Medical Center (PIMC).

• Provide a Blood Gas Analyzer instrument capable of fully automated, on-demand arterial, venous, and pleural blood gas testing.

• The instrument must measure pH, PC02, P02, S02, tHb, F02Hb, FCOHb, and FMetHb.

• Key instrument features include: fully automated sampling and mixing, automatic quality controls, automatic calibrations, and a non-cartridge-based system.

• Provide Data Management software with all upgrades and license renewals for the lease term.

• Deliver reagents and supplies for approximately 800 tests per year, with quarterly delivery and expedited delivery available when needed.

• Provide installation of the analyzer and training for PIMC Respiratory Therapy staff at no additional cost.

• Provide a warranty covering labor, travel, parts, and software upgrades for the lease term.

• Calibrations occur every 30 minutes.

• Automatic Quality Control levels every eight (8) hours.

• Blood Gas Lab services are available 24 hours/day, 7 days/week.

• Notification required to IHS for items on backorder for more than 7 days.

• Notice in writing to the Contracting Officer Representative (COR) for any changes in product availability, production, or specifications.

• Ability to fulfill urgent delivery requirements for overnight delivery.

• Allow return or full credit for items with unreasonable shelf life; return shipping is the contractor's responsibility.

• Lease Agreement for one (1) base year plus four (4) option years (total of five years).

• Delivery of the Blood Gas Analyzer to Phoenix Indian Medical Center, 4212 N. 16th Street, Phoenix, Arizona, 85016, within 30 days of award.

• The instrument must include Data Management software.

• Warranty must include and cover labor, travel, parts, and software upgrades at no cost to the Government.

• Contractor must provide Quality Controls, Reagents, and Supplies for 800 tests per year.

Sources Sought Notice

This notice is for market research purposes to determine the availability of Indian Economic Enterprises (IEE) and Indian Small Business Economic Enterprises (ISBEE) capable of fulfilling a requirement for a five-year lease agreement for a Blood Gas Analyzer. The primary goal is to assess compliance with the Buy Indian Act and inform a potential set-aside decision.

The requirement is for a Blood Gas Analyzer capable of performing fully automated, on-demand testing and measurement of arterial, venous, and pleural pH, PCO2, PO2, SO2, tHb, FO2Hb, FCOHb, and FMetHb. The lease must include:

• Data management software

• Installation

• Staff training

• Warranty coverage

• Maintenance

• Quality controls

• Reagents

• Supplies

• Technical support

• Support for approximately 800 tests annually.

• The anticipated contract type is a five-year lease agreement.

• Place of Performance: Phoenix Indian Medical Center, Phoenix, AZ.

This is NOT a Request for Quote (RFQ) or Request for Proposal (RFP). Interested parties must respond in writing with specific information to demonstrate their qualifications and eligibility under the Buy Indian Act. Required information includes:

• Company name

• Point of contact details

• FSS / SEWP Contract number (if applicable)

• DUNS number or SAM Unique Entity Identifier

• Tax ID number

• Business category (IEE or ISBEE)

• Capability statement

• Manufacturer authorization documentation

• Completed IHS Indian Economic Enterprise Representation form

• Identification of the Federally Recognized Indian Tribe or Alaska Native Corporation for IEE status.

• The IHS Contracting Officer will give priority for purchases to ISBEE set-asides.

• Acquisitions subject to commercial items or SAP must be set-aside exclusively for ISBEEs.

• Definitions for Indian Economic Enterprise (IEE) and Indian Small Business Economic Enterprise (ISBEE) are provided.

• Responses are due by 6/15/2026 at 1300 ET.

• Firms must be registered in SAM and complete electronic annual representations and certifications.
