Sources Sought Notice

This notice is a Sources Sought for market research to determine compliance with the Buy Indian Act and to aid in the determination of a set-aside for the procurement of maintenance services for EchoSens FibroScan MINI 430+ equipment. This is NOT a Request for Quote.

• Maintenance Service for EchoSens FibroScan MINI 430+ equipment.

• The procurement is for new OEM medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (i.e., replacement parts).

• Gray market items and remanufactured items will NOT be acceptable.

• Vendors must be an OEM, authorized dealer, authorized distributor, or authorized reseller, verified by an authorization letter or other OEM documentation.

• All software licensing, warranty, and service must be in accordance with OEM terms and conditions.

• Place of Performance: Phoenix Indian Medical Center, 4212 N 16th St, Phoenix, AZ 85016.

• Response Date/Time: 03/11/2026, 1300 Local Time.

• Interested firms must respond in writing.

• Responses must include: Company name, Point of contact, FSS/SEWP Contract number (if applicable), DUNS or SAM Unique Entity Identifier, Tax ID number, indication of business category (IEE or ISBEE), a capability statement, and documentation from the manufacturer verifying authorized distributor status.

• The government will use responses to make an appropriate acquisition decision.

• The IHS Contracting Officer will give priority for all purchases to Indian Economic Enterprises (IEE) and Indian Small Business Economic Enterprises (ISBEE) under the Buy Indian Act.

• Vendors must self-certify eligibility on the designated IHS Indian Economic Enterprise Representation form.

• Vendors must identify the Federally Recognized Indian Tribe or Alaska Native Corporation upon which they rely for IEE status.

• Prospective contractors must be registered in the System for Award Management (SAM).

• This is a Sources Sought Notice only; it is not a Request for Quote.

This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" (IEE) as required by the Buy Indian Act. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Solicitation/Sources Sought/RFI #: Field to enter the relevant identifier.
• Project Title: Field to enter the project's title.
• Date: Field to enter the date.
• Representation: Offerors must indicate whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act.
• Name of Federally Recognized Tribal Entity: Field for the name of the tribal entity.
• Name of the 51% owner of IEE Firm: Field for the name of the primary owner.
• Business Name: Field for the business's name.
• DUNS Number: Field for the Data Universal Numbering System number.
• Certifying Signature: Field for the signature of the authorized representative.
• Print Name: Field for the printed name of the signatory.

Bidders must complete and submit this form to self-certify their eligibility as an Indian Economic Enterprise. Failure to meet the definition or properly complete the form may affect eligibility for contracts set aside under the Buy Indian Act. The form also outlines the periods during which the enterprise must meet the IEE definition (offer submission, award, and contract performance) and the requirement to notify the Contracting Officer if eligibility is lost. It also notes that successful offerors must be registered in the System of Award Management (SAM).
