Statement of Work (SOW) / Performance Work Statement (PWS)

To obtain reliable, timely, and cost-effective maintenance and repair services for critical government-owned sterilization equipment at three dental clinics operated by the Moncrief Army Health Clinic (MAHC) at Fort Jackson, South Carolina. The goal is to maximize equipment uptime, ensure operational reliability, and reduce total cost of ownership through a scheduled preventive maintenance program.

• Provide comprehensive preventive maintenance, unscheduled repair, and related support services for sterilization and decontamination equipment.

• Cover equipment located at Hagan, Oliver, and Caldwell dental clinics.

• Includes providing all necessary personnel, supervision, tools, materials, equipment, transportation, and Original Equipment Manufacturer (OEM) parts.

• Task 1: Perform Scheduled Preventive Maintenance (PM): Conduct PM on all listed equipment according to OEM's most current recommended service procedures and intervals. Coordinate with the clinic POC at least 30 days in advance.

• Task 2: Provide Unscheduled Corrective Maintenance (Repairs): Diagnose and repair malfunctioning or non-operational equipment, including routine and emergency service calls.

• Task 3: Deliver Operator Training: Provide operator-level training for up to five government personnel per clinic on equipment operation, care, and basic troubleshooting.

• Task 4: Contract Administration and Reporting: Provide detailed service reports for all maintenance actions.

• Scheduled PM: 100% completed within the required timeframe.

• Routine Repair Calls: 95% of calls must meet the on-site response standard (4 business hours for initial contact, 24 business hours for technician on-site).

• Emergency Repair Calls: 100% of calls must meet the on-site response standard (1 hour for initial contact, 24 hours for technician on-site).

• Operator Training: 100% compliance.

• Service Reports: 95% submitted on time per month.

• Place of Performance: Hagan, Oliver, and Caldwell Dental Clinics, Fort Jackson, SC.

• Hours of Operation: Normal hours for scheduled services are 0730-1630 Monday-Friday, excluding Federal holidays. Unscheduled emergency services are required during regular business hours.
