This is a combined synopsis/solicitation (RFQ 36C25926Q0474) for commercial items, specifically surgical monitors, for the Oklahoma City Veterans Health Care System. The government anticipates awarding a firm-fixed-price contract.

• Item 0001: Five (5) 31.5” 4K Mini-LED Surgical Displays with specific technical specifications (3840 x 2160 resolution, 1000:1 contrast, 5 MS response time, 800 cd/m2 brightness, multiple input/output ports including SDI (12G/3G), weighing 21.4 lbs, with domestic end-product warranty.

• Item 0002: Six (6) 55” LED-backlit LCD TVs (Domestic End Product).

• The scope includes providing a comprehensive solution, potentially including items not explicitly listed but necessary for functionality (e.g., installation expenses, VA staff training, mounts, cables, connectors). The contractor must be an expert in hospital OR/ICU rooms.

• Equipment must be the newest and most current model from a manufacturer of record.

• "White glove" installation is required for all products.

• Installation of inventory management software on a government desktop/laptop and system setup on a government laptop.

• Preparation of a government-owned server for contractor-government data transmission for maintenance and monitoring.

• Contract Type: Firm-fixed-price.

• Period of Performance: 45 days from award date for delivery.

• Warranty: One-year manufacturer's free replacement warranty with free return shipping.

• Questions Deadline: May 18th, 2026, 3:00pm MST (submit in writing).

• Quote Deadline: May 20th, 2026, 3:00pm MST (emailed to luther.dickens@va.gov; 4MB size limit per email).

• Evaluation: Award will be made to the responsible offeror whose offer is most advantageous to the Government, considering price only. Offers must meet technical capability requirements. Award will be made on an All or Nothing basis.

• This solicitation is 100% set-aside for Service-Disabled Veteran-Owned Small Business (SDVOSB) concerns.

• SDVOSB status must be verified in the Veteran Small Business Certification Pages (Vert-Cert) at the time of quote submission and award.

• Prohibition: No remanufactured or "gray market" items will be acceptable. Offerors must be an OEM, authorized dealer, distributor, or reseller, verified by the OEM.

• Solicitation provisions and clauses are effective through Federal Acquisition Circular (FAC) 2026-01.

• NAICS Code: 334118, Small Business Size Standard: 1,000 EMP.

• Delivery/Installation Address: Oklahoma City – OR & SICU Renovation, 921 NE 13th Street, Oklahoma City, OK 73104.

• Delivery is FOB Destination.

• Offerors must complete and sign the RFQ entirely.

• Offerors are warned against contacting VA personnel other than the Contracting Officer and Contract Specialist.

This document is the VA Notice of Limitations on Subcontracting - Certificate of Compliance for Supplies and Products (NOV 2022). It requires offerors to certify their compliance with subcontracting limitations if awarded a contract.

• Subcontracting Limitations: Offerors must select the appropriate box to certify compliance with limitations on subcontracting. This includes:
  • For contracts for supplies (not from a non-manufacturer): Not paying more than 50% of the contract amount to firms that are not VIP-listed SDVOSBs or VOSBs. Costs of materials are excluded.
  • For contracts for supplies from a non-manufacturer: Supplying products of a domestic small business manufacturer or processor, unless a waiver is granted.
• Manufacturer or Nonmanufacturer Representation: Offerors must certify whether they are the manufacturer/producer of the end item (and if it's domestically produced) or if they qualify as a nonmanufacturer (meeting specific employee count, business type, and ownership/possession criteria).
• Certification of Compliance: A formal certification statement must be completed, signed, and returned with the offer.
• Penalties: False certifications are subject to penalties under Title 18, United States Code, Section 1001, and potential referral to the VA Suspension and Debarment Committee.
• Cooperation: Offerors agree to provide documents to VA to verify compliance.

This is a mandatory certification. Bidders must complete and return this form with their offer. Failure to provide the certification will render the offer ineligible for evaluation and award. It requires careful review to ensure the bidder's business model and subcontracting plans align with the specified limitations for SDVOSB/VOSB set-aside contracts.
