This is a combined synopsis/solicitation (RFQ 36C25926Q0474) for commercial items, specifically Surgical Monitors for the Oklahoma City Veterans Health Care System's OR & SICU Renovation. The government anticipates awarding a firm-fixed-price contract. This solicitation is 100% set-aside for Service-Disabled Veteran-Owned Small Business (SDVOSB) concerns.

• Item 0001: 31.5” 4K with Mini-LED Surgical Display (Qty: 3)

• Item 0002: 55” LED-backlit LCD TV (Qty: 3)

• The scope includes providing brand-name or equal surgical monitors, installation, performance verification, white glove service, software installation on government computers, and preparation of a government server for maintenance and monitoring.

• A one-year manufacturer's warranty is required, including free replacement and return shipping.

• The contractor must provide a comprehensive solution, including any necessary items not explicitly listed (e.g., cables, connectors, mounts, installation expenses, training).

• Equipment must be the newest and most current model from a manufacturer of record.

• No remanufactured or gray market items will be accepted.

• Contract Type: Firm-fixed-price.

• Period of Performance: 45 days from the award date for delivery.

• Questions Deadline: June 12th, 2026, 3:00pm MST (submit in writing).

• Quote Deadline: June 15th, 2026, 3:00pm MST (email to luther.dickens@va.gov).

• Quotes must be emailed (4MB size limit per email); no fax or hand-carried quotes accepted.

• Award will be made on an "All or Nothing" basis.

• Evaluation Criteria: Price only, provided the offer meets technical capability requirements.

• 100% set-aside for SDVOSB concerns.

• SDVOSB status must be verified in the Veteran Small Business Certification Pages (Vert-Cert) at the time of quote submission and award.

• Offerors must be an OEM, authorized dealer, distributor, or reseller, verified by the OEM.

• The solicitation incorporates FAR Part 8 and other FAR/VAAR clauses as listed.

• Delivery is FOB Destination to the Oklahoma City – OR & SICU Renovation location.

• Offerors are warned against contacting VA personnel other than the Contracting Officer or Contract Specialist.

This document is the VA Notice of Limitations on Subcontracting - Certificate of Compliance for Supplies and Products. It requires offerors to certify their compliance with subcontracting limitations if awarded a contract. Bidders must complete and return this certification with their offer.

• Limitations on Subcontracting: Offerors must select the appropriate box indicating compliance for:
  • Contracts for supplies (not paying more than 50% to non-VIP-listed SDVOSBs/VOSBs).
  • Contracts for supplies from a non-manufacturer (must supply products from a domestic small business manufacturer or processor, unless a waiver is granted).
• Manufacturer or Nonmanufacturer Representation: Offerors must certify whether they are the manufacturer/producer or qualify as a non-manufacturer, including specific employee count, business type, and ownership/possession requirements for non-manufacturers.
• Certification of Compliance: A formal certification statement must be signed by an authorized representative.
• Penalties: Acknowledges that false certifications are subject to penalties under Title 18, United States Code, Section 1001, and may lead to suspension, debarment, fines, or prosecution.
• Cooperation: Agreement to provide documents to VA for compliance verification.

This is a mandatory certification. Failure to complete, sign, and return this document with your bid, quotation, or proposal will render your offer ineligible for evaluation and award. Bidders must carefully review and accurately complete the certification based on their company's status and intended subcontracting plan.
