Statement of Work (SOW)

To provide full leasing, preventative maintenance, and repair services for surgical scopes for the Eastern Oklahoma VA Health Care System (EOVAHCS), specifically for the new Tulsa VA Medical Center and other locations within EOVAHCS.

• Lease of surgical scopes and accompanying equipment.

• Comprehensive service agreement for preventative maintenance and repair.

• Services to be performed by Original Equipment Manufacturer (OEM) certified personnel.

• Equipment must meet or exceed specified physical, functional, and performance characteristics.

• Includes specific equipment sets for Urology/ENT, Sinus, Panendoscopy, Flexible Scopes, Weerda Diverticuloscope, Peel Pack Items, Flexible Cystoscopes, Perc Neph Short/Long, Semi-Rigid Scopes, Rigid Cystoscopes, Cysto Sets, Generator, DVIU, Resection, and ENT procedures.

• Includes a "SLIM DESIGN SURGICAL CART W/OPEN SIDES KIT WITH CO2 TANK HOLDER XUSM0008".

• StreamConnect services for Tulsa VAMC and Muskogee VAMC.

• Contractor must provide products and perform in a professional manner recognized by commercial industry standards.

• Contractor must provide written certification from the OEM authorizing them to provide and assemble products.

• Contractor must have the ability to repair/service equipment per OEM specifications to avoid voiding warranty.

• Equipment service reports must be submitted to the Contracting Officer's Representative (COR).

• Performance will be monitored for timeliness of repair and completion of services.

• Base period of performance: September 1st, 2026, to August 31st, 2027.

• Delivery of products required no later than 100 days post contract award.

• Locations include Tulsa VAMC and Muskogee VAMC.

• Brand Name or Equal selection required.

• Contractor must provide assembly and certified in-service training for relevant staff at no additional cost.

• Electronic copies of user and repair manuals must be provided.

• Adherence to local, state, and federal safety, health, and environmental guidelines.

• Compliance with specific regulatory requirements (EPA, DOT, OSHA, VHA Directives).

• Contractor must provide a Contractor Point of Contact (POC).

• Contractor must provide adequate liability insurance.

• Contract personnel must adhere to security requirements, including check-in/out and escorting on VA premises.

• Warranty must meet or exceed standard manufacturer industry terms, beginning August 1st, 2026.

Request for Information (RFI)

This RFI is issued by the Department of Veterans Affairs (VA) for market research purposes to identify contractors capable of providing surgical scope leasing, preventative maintenance, and repair services for the Eastern Oklahoma VA Health Care System (EOVAHCS) in Tulsa, Oklahoma. This is not a solicitation or a Request for Quote (RFQ).
