• System Identification: Details about the medical device/equipment, including title, version, description, model, alternate names, category, software EOL, regulatory certifications (FDA 510K), existing contracts, DoD/SMD Authorization status, physical testing location, mode of operation (e.g., Standalone, Client/Server, Cloud-based), cloud offerings, method of implementation (hardware/software/firmware), interfaces (LAN connection type), and connected systems.
• Technical Information: Comprehensive details on system hardware/firmware, operating systems (including EOL dates and virtual environment status), installed applications (including EOL dates and purpose), migration paths for unsupported software, user account types and authentication methods, system architecture diagrams, endpoint protection (Antivirus/HBSS/HIPS) capabilities, vulnerability scanning policies and limitations, vulnerability management/patching policies, remote access capabilities, wireless capabilities (802.11, Bluetooth, other), and mass storage usage (USB drives).
• Data Processing Capabilities: Information on data processing functions (Receive, Process, Store, Transmit, Display), handling of Electronic Protected Health Information (ePHI)/Personally Identifiable Information (PII), specific ePHI/PII elements processed, data persistence and removal methods, data input methods, data workflow, Data At Rest encryption, and Data in Transit encryption.

Completion of this questionnaire is critical for vendors proposing medical devices or equipment. Failure to disclose required information or misrepresentation can result in the system being ineligible for purchase or lead to contract breach and cancellation. The detailed technical and security information requested directly impacts the RMF Authorization process and the overall eligibility of the proposed system for DoD use. Vendors must address all information fields and cannot rely on external documents or product literature.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial products or services, issued as a Request for Quote (RFQ) under FAR subpart 12.6 and FAR 13 (Simplified Acquisition Procedures). The Defense Health Agency Contracting Activity (DHACA) is procuring Surgical Sterilizer Test Articles on behalf of the Army Program Executive Office (PEO) Soldier, Soldier Medical Devices (SMD) Project Management Office (PMO).

• The primary requirement is for Surgical Sterilizer Test Articles.

• Additional items include Onsite In-Person Training.

• An option line item for additional Surgical Sterilizer Test Articles is also included.

• The initial procurement is for four (4) surgical sterilizers, with options for three (3) additional units.

• A subsequent sole source procurement for up to 711 surgical sterilizers over five years is anticipated after operational and environmental testing.

• Quotes are due by 10:00 AM Local Time on January 30, 2026.

• Quotes should be submitted as Microsoft Word, Microsoft Excel, or Adobe PDF attachments.

• Questions concerning the RFQ are due by 4:00 PM Local Time on January 2, 2026, via email.

• Evaluation will be based on a competitive best value tradeoff among price and non-price factors, including Technical Capabilities/Specifications, Risk Management Framework, Past Performance, and Price.

• Past Performance is more important than Price. Non-price factors are collectively more important than price.

• Quotes must be legible and comprehensive.

• This requirement is being solicited full and open.

• Applicable FAR and DFARS clauses are incorporated by reference and in full text.

• Contractors must be registered in the System for Award Management (SAM) database at the time of award.

Lists Sheet: Contains predefined lists for Hardware Type, Software Type, and Approval Status, which can be used for dropdown selections within the template.

• Evaluation Factor I – Technical Capabilities/Specifications: Defines adjectival ratings from "Outstanding" to "Unacceptable" based on the approach, understanding of requirements, and risk of unsuccessful performance.
• Evaluation Factor II – Risk Management Framework: Defines "Acceptable" and "Unacceptable" ratings based on whether the quote meets solicitation requirements.

• Support of Device: For future purchases, ensure procurement for a minimum of five years and support for parts for an additional five years; provide supportability, maintenance, technical documentation, and OEM/government-provided sustainment support.
• Defense Logistics Agency, Electronic Catalog (ECAT): Willingness to work with DLA ECAT or a Prime Vendor to add the device to ECAT for streamlined procurement.
