Scope of Work

The requirement encompasses two primary locations and scopes:

• Uintah and Ouray Health Clinic: Provide and install new system furniture, reconfigure existing space, extend phone and data connections, and include electrically assisted adjustable sit-to-stand workstations. Work must be coordinated with clinic staff and occur after hours or on weekends.
• JWIC Building: Convert the fitness area into an office workspace, provide additional furniture, and install a new furnace. Specific layouts are provided in the "drawing furniture jwic" attachment.

General requirements for both projects include:

• Procurement of NEW Equipment ONLY; no remanufactured or "gray market" items.
• Vendor must be an Original Equipment Manufacturer (OEM) authorized dealer, distributor, or reseller.
• Items are to be shipped FOB Destination, with shipping costs included in the vendor quote.
• Adherence to the International Building Code (latest edition) and OSHA safety regulations.
• Contractor is responsible for disposal of all project-generated debris.

• Evaluation Criteria: Award will be made to the responsive and responsible vendor providing the Lowest Price Technically Acceptable (LPTA). Evaluation factors include Price, Technical Acceptance of Product, and Delivery. Past performance will be evaluated using PPIRS and FAPIIS data.
